| Objective:Through periodontal examination and questionnaire survey of 600 community elderly people in taiyuan urban area,and statistical analysis of the survey results,the risk factors of periodontitis affecting the community elderly people in this area were obtained,an important theoretical basis for promoting oral health of the elderly population in this area were provided.Methods:The questionnaire survey and periodontal examination were conducted among 600 community elderly aged 65 to 74 in taiyuan city.Among them,the questionnaire mainly involves social and demographic characteristics(gender,age,nationality,occupation,education background,income,etc.);Systemic diseases(diabetes,cardiovascular diseases,digestive diseases,tumors,etc.);Oral health habits(frequency,manner,length of brushing,after-meal gargling habit,cleaning history,etc.);Smoking status(whether or not you smoke,number and frequency of smoking per day,age of smoking,smoking cessation,etc.);Periodontitis cognitive status(do you know about periodontitis? Are bad teeth natural? Is person old tooth normal phenomenon? Is bleeding from brushing normal?Are diabetes and cardiovascular diseases related to periodontitis? Does smoking aggravate periodontal inflammation? Does the toothbrush need to be changed regularly?Will cleaning damage the teeth and widen the gap? Do teeth need regular examination? Is a denture necessary? Behavior of seeking medical treatment(whether to see a doctor for loose tooth bleeding,the main reason for not seeking medical treatment,payment methodof medical treatment fee,etc.)At the same time,the Community Periodontal Index(CPI),Calculus Index(DI),Calculus Index(CI),and Periodontal conditions of the patients were checked.A total of 581 valid questionnaires and corresponding checklists were sorted out and input into the Excel form.SPSS23.0 software was used for statistical analysis of the factors that might affect the prevalence of periodontitis in the elderly community in taiyuan city.The data of bicategorical variables(gender,oral hygiene habits,dental cleaning history,diabetes,cardiovascular disease,smoking,economic status,and medical treatment)were tested by four-square table 2 test.Data of multicategorical variables(education level,cognitive status)were tested by independent sample nonparametric test.Multivariate analysis was performed by binary categorical non-conditional Logistic regression analysis,and P < 0.05 was considered as the test level.Results:1.The prevalence of periodontitis is high in taiyuan community.2.Oral health habits of the elderly in taiyuan community: 59.0% of the elderly brushed their teeth less than twice a day,and 93.1% of the elderly never cleaned their teeth.Cognitive status: 91.3% of the elderly think that aging is a normal phenomenon,82.1% of the elderly do not know that diabetes,cardiovascular disease and periodontitis is closely related,80.7% of the elderly do not know that smoking and periodontitis is related,43.3% of the elderly think brushing bleeding is a normal phenomenon.Behavior of seeking medical treatment: only 24.4% of the elderly choose to seek medical treatment due to periodontal symptoms,among which 71.1% and 54.4% think that bleeding gums and loose teeth do not need medical treatment respectively.3.Univariate analysis showed that gender(P=0.047),education level(P=0.007),oral hygiene habits(P=0.003),dental cleaning history(P=0.001),diabetes mellitus(P=0.000),smoking(P=0.000),cognitive status(P=0.038),and medical behavior(P=0.000)were significant.Logistic regression analysis results of the above factorsshowed that: Diabetes mellitus(OR value :3.267,OR95%CI:1.635-6.529),smoking(OR value :2.687,OR95%CI:1.791-4.032),medical behavior(OR value :0.391,OR95%CI:0.212-0.721),cognitive status(OR value :0.275,OR95%CI:0.126-0.597),education level(OR value: 0.365,OR95%CI: 0.228-0.585)entered the equation.Conclusion:1.The prevalence of periodontitis in taiyuan community is higher than the national average2.Most of the community elderly in this area have not mastered the correct oral health,lack of awareness of periodontitis and do not actively seek medical treatment.3.Diabetes mellitus,smoking,education,medical behavior and cognitive status were the influencing factors. |
