Objective:The clinical efficacy and adverse reactions of 70 patients with hepatic failure treated by plasmapheresis(PE)were retrospectively reviewed and analyzed,in order to provide a theoretical basis for the treatment of patients with liver failure.?Methods:Seventy patients with liver failure who were hospitalized with PE in our hospital from June 2015 to June 2018 were enrolled.PE was performed on the basis of conservative treatment of internal medicine. The clinical symptoms, main biochemical indicators and adverse reactions during the treatment were compared and analyzed.PE was compared and analyzed for patients with different stages, types and different causes of liver failure. The clinical efficacy,as well as the four-year survival of patients after PE treatment?Results:1.The effective rate of 70 patients with liver failure after PE treatment was 54.29%.Alanine aminotransferase(ALT),aspartate aminotransferase(AST),serum total bilirubin(TBIL),prothrombin time(PT)levels in patients after treatment compared with before treatment Significantly decreased, prothrombin activity(PTA)was significantly increased, the difference was statistically significant.2.PE treatment was effective in patients with early liver failure,up to 83.3%;the efficacy of patients with intermediate and advanced liver failure decreased in turn, andthe difference between the early liver failure group and the middle-end group was statistically significant.3.The relationship between PE efficacy and different types of liver failure: After PE treatment, the effective rate of patients with acute liver failure(70%)was higher than that of subacute liver failure(50%)and chronic liver failure(52%).After Fisher's exact probability test,P>0.05 was measured, and the difference was not statistically significant.4.The relationship between the efficacy of PE and the etiology of liver failure: For patients with liver failure, the effective rates of PE treatment for hepatitis B liver failure,drug-induced liver failure, and alcoholic liver failure were 44.83%,52.94%,and 71.43%,respectively.There was no significant difference in clinical efficacy between the groups(P>0.05).5.A total of 70 patients with liver failure underwent PE treatment for 156 times, and a total of 23 adverse reactions(14.7%)occurred.No serious adverse reactions such as bleeding or shock occurred. Conclusion:PE treatment can effectively improve the clinical symptoms and laboratory indicators of patients with liver failure, and there is no obvious adverse reaction.Early treatment with PE in patients with liver failure can achieve better results. There was no difference in the efficiency of PE patients treated with different types and different causes of liver failure.
