Objective: to investigate the effect of astragalus injection on distal limb blood flow after femoral artery anastomosis in rabbits.By artificially creating a model of arterial crisis after vascular anastomosis and using Chinese herbal preparations for external intervention,it is proved that Chinese herbal preparations have good pharmacological effects on alleviating arterial crisis.Methods: 48 New Zealand white rabbits were randomly divided into 3 groups(n=16): model group(n=16),traditional Chinese medicine group(n=16)and western medicine group(n=16).After the femoral artery on both sides of the rabbit was fully exposed,the femoral artery was cut off artificially,and then the broken end of the artery was sutured under the microscope with microsurgical suture technology,and the broken end of the artery was subjected to pressure cold stimulation with ice cubes,so that the blood vessels at the anastomosis of the artery significantly contracted,presenting the model state of arterial crisis.In the model group,0.9% sodium chloride injection was used for continuous local infusion of anastomotic vessels with vasospasm.In the western medicine group,papaverine hydrochloride injection and 0.5% sodium chloride injection were diluted to 5g/ml for continuous infusion at the anastomotic site with vasospasm.In the traditional Chinese medicine group,astragalus injection was used to continuously drip the anastomotic site.(1)observe and measure the inner diameter of blood vessels at the anastomotic site before and after medication at 2min,4min and 8min under the microscope;(2)laser doppler flowmeter was used to monitor the blood flow velocity of capillaries at the same point in the distal limb before and at 24 h and 48 h after surgery.(3)plasma APTT and FIB values were measured before and after medication at 24 h and 48 h by automatic coagulation analyzer.(4)after HE staining,the infiltration of inflammatory cells on the endothelial surface of the three groups was observed under electron microscope 48 h after surgery.Results :(1)at 2min,4min and 8min,the measured vascular diameter in the model group gradually widened,but the rate of change was small.The measured vascular diameter in the TCM group showed obvious changes in the early stage,and reached the maximum diameter at the 4th min.The inner diameter of blood vessels in the western medicine group reached the maximum at 2min and slightly decreased at 4min.There was no significant difference between traditional Chinese medicine group and western medicine group(P>0.05).The comparison between the traditional Chinese medicine group and the model group was statistically significant(P<0.05).The comparison between western medicine group and model group was statistically significant(P<0.05).(2)both the papaverine hydrochloride injection group and the astragalus injection group 24 h after surgery were significantly higher than the 0.9% sodium chloride injection group,with statistical significance(P < 0.05).The difference between astragalus injection group and papaverine hydrochloride injection group was not statistically significant(P > 0.05).48 h after surgery,both the papaverine hydrochloride injection group and the astragalus injection group were significantly higher than the 0.9% sodium chloride injection group,and the difference was statistically significant(P<0.05).The difference between papaverine hydrochloride group and astragalus injection group was not statistically significant(P>0.05).(3)the comparison between two groups,model group,respectively,compared with western medicine,Chinese medicine group,the differences were statistically significant(P < 0.05).At 24 h and 48 h after surgery,the APTT of coagulation factor in the TCM group was lower than that in the western medicine group,and the FIB in the TCM group was higher than that in the western medicine group,and the comparison between the two groups was also statistically significant(P<0.05).(4)in 48 h,three groups of endothelium were observed under electron microscope,the results of astragalus injection group of inflammatory cells infiltrating the least,and astragalus injection group organization dehydration,few apoptotic cells appeared,papaverine hydrochloride injection group,saline group,comparison between the three groups,Chinese traditional medicine astragalus membranaceus injection is superior to western medicine group and model group.Conclusion :(1)both the western medicine papaverine injection and astragalus injection can improve the blood flow at the distal end of the anastomosis after femoral artery anastomosis in rabbits,but the comprehensive effect of astragalus injection is better.(2)the principle of astragalus injection to increase the blood flow at the broken end may be related to dilating the peripheral arterioles,relieving vasospasm,reducing the aggregation of coagulant substances and inhibiting inflammatory factor infiltration.
