Therapeutic Effect Of Sophorae Flavescentis Radix Gelatum On Aerobic Vaginitis Combined With Bacterial Vaginosis

Objective: Aerobic vaginitis(AV)combined with bacterial vaginosis(BV)is one of the most common vaginal infections in gynecological clinics.Both AV and BV are caused by vaginal microecological disorders leading to over-reproduction of conditional pathogens.There is currently no standardized treatment for AV.Although BV has a guiding treatment plan,the recurrence rate is higher.This study was designed to evaluate the efficacy of sophorae flavescentis radix gelatum in the treatment of AV combined with BV and explore the treatment options for AV+BV.Methods: From September 2017 to January 2019,the patients diagnosed with AV+BV according to the AV Donders score and Nugent score at the General Gynecology Clinic of Tianjin Medical University General Hospital were used as prospective,randomized,controlled clinical trials.Randomized to give AV+BV patients 3 different treatment options(sophorae flavescentis radix gelatum group: 1 per night of sophorae flavescentis radix gelatum into the depth of the vagina,8 days for a course of treatment;metronidazole group: oral metronidazole 400 mg,3 times/day,for 7 days;metronidazole + sophorae flavescentis radix gelatum group: oral metronidazole 7 days,while vaginal sophorae flavescentis radix gelatum for 8 days),compare the microbiological effects of the three treatment options.Clinical efficacy and recurrence rate were evaluated to assess the safety of each protocol.Comparison of categorical variables was performed using chi-square test or Fisher exact probability method.The rank sum test was used to compare the grade data.Results: Of the 95 patients who were treated with intention,10 patients were absent,and 85 patients participated in the first follow-up.Among them,there were 22 cases in sophorae flavescentis radix gelatum group,the cure rate was 40.9%(9/22),the effective rate was 36.4%(8/22),and the ineffective rate was 22.7%(5/22);there were 25 cases in metronidazole group,the cure rate was 56.0%(14/25),the effective rate was 36.0%(9/25),and the ineffective rate was 8.0%(2/25);there were 38 cases in metronidazole + sophorae flavescentis radix gelatum group,the cure rate was 68.4%(26/38),the effective rate was 21.1%(8/38),and the ineffective rate was 10.5%(4/38).The cure rate of metronidazole + sophorae flavescentis radix gelatum group was higher than that of metronidazole group and sophorae flavescentis radix gelatum group,but there was no significant difference between the three groups(P>0.05).At the second follow-up,the recurrence rate of the sophorae flavescentis radix gelatum group was 42.9%,the recurrence rate of metronidazole group was 37.5%,and the recurrence rate of metronidazole + sophorae flavescentis radix gelatum group was 29.4%.There was no significant difference in the recurrence rate between the three groups(P>0.05).Conclusion: This study shows that the simple use of sophorae flavescentis radix gelatum,oral metronidazole and sophorae flavescentis radix gelatum combined with metronidazole have a certain therapeutic effect on patients with AV combined with BV,in which metronidazole combined with sophorae flavescentis radix gelatum for AV combined with BV has the best efficacy and the lowest recurrence rate.The sophorae flavescentis radix gelatum combined with metronidazole is superior to the two monotherapy groups in restoring vaginal lactobacillus.
