Objective To investigate the effect of three-dimensional power Doppler transvaginal ultrasound(3DPD-TVS)quantitative analysis of ovarian masses perfusion parameters and evaluate its value for identifying the malignant and benign ovarian masses.Methods Ovarian masses were examined with conventional ultrasound and 3DPD-TVS.Four parameters including the resistance index(RI),vascularization index(VI),flow index(FI),and vascularization flow index(VFI)were analyzed by comparing the area under the ROC curves with date postoperative pathological results.The diagnosis values of VI,FI,and VFI are investigated by using the whole mass and 10cm3 with the most abundant flow as the regions of interest.Results VI,FI,and VFI values of the malignant group are significantly higher than the benign group,the difference between the two groups are statistically significant(P<0.05).The areas under VI,FI,and VFI curves are 0.820,0.727,and 0.838 respectively.When the cutoff value of VFI is 0.436,its sensitivity is 89.8% and the specificity is 68.0%.RI showed no significant difference between the malignant group and the benign group(P>0.05).By selecting the most abundant ovarian mass flow 10cm3 region of interest analysis we found there were significant differences in VI,FI and VFI values in malignant group,borderline group,benign group(P<0.05),malignant group was higher than that in the borderline group,and the borderline group was higher than that in the benign group.Conclusions 3DPD-TVS parameters including VI,FI,and VFI are highly valuable for the identification of benign and malignant ovarian masses.Among them,VFI is the most valuable and its cutoff value is 0.436.Select the most abundant ovarian mass flow 10cm3 region of interest analysis can improve the diagnostic accuracy of borderline ovarian tumor,provide a great value for early diagnosis of ovarian malignant tumor.
