Objective: To analyze the treatment options and evaluate the therapeutic effect of adult diacapitular condylar fractures(DCFs)by a retrospective study.Methods: This study was divided into three parts.In the first part,epidemiological analysis was on the adult DCFs and to understand the characteristics of adult DCFs.In the second part,we introduced the surgical treatment of adult condylar fracture and evaluate its curative effect and then compared the two different surgical treatment methods,the traditional surgical method and the modified surgical method,and get to know which one is better for the adult DCFs.In the third part,we compared the effect and prognoses of the conservative treatment and the surgical treatment,to get which one is better for the adult DCFs.Results: 1.There was no significant gender difference in adult condylar fractures.The age between 18 and 44year-old was the most frequent age that the adult DCFs occurred,and the most frequent injury cause was fall injury.The unilateral fracture was more frequent than the bilateral and the most frequent associated fracture region was symphyseal and parasymphyseal.The type B was the most common fracture type and most of them were obtained surgical treatment.2.1 We found that the displaced TMJ disc during the DCFs exhibited similar displace direction of the condylar head,and the open reduction and internal fixation combined with the disc anchorage was the best method for the adult DCFs.2.2 For the modified surgical method,joint disc anchorage was combined with internal fixation of fracture,which was more effective than the traditional surgical treatment.3.Open treatment was easily to obtain better clinical prognosis,but the conservative treatment was relatively limited,and the effect was worse.Conclusion: The male was more frequent than the female,the middle-young aged was in majority and the most common reason of the fracture was fall injury.Unilateral fracture was in majority and the associated fracture region was symphyseal and parasymphyseal.The modified suigical technique was more effective than the traditional one,and the surgical treatment was more effective than the conservative one.
