Objective:The serum S100B calcium binding protein B(S100B),neuron-specific enolas(NSE),hearing screening,head imaging examination and neurobehavioral assessment of Transient hypothyroxemia(THOP)were carried out.To analyze the correlation of transient hypothyroidism with brain injury and neurobehavior.Methods:80 premature infants admitted to the neonatal department of Tongliao city hospital from September 2017 to December 2018 were selected as the research object.According to the thyroid function evaluation on the 7th day of admission,40 premature infants with transient hypothyroxinemia were selected into the study group,and 40 premature infants with normal thyroid function were selected into the control group.All the premature infants in the study group and the control group were took 4ml venous blood on the 7th day after birth to detect thyroid function,serum NSE and S100B protein.The initial hearing screening was performed on the 3rd to 5th day after birth.The review of hearing screening was performed on the 42nd day after birth.The Neonatal Intensive Care Unit Network neurobehavioral Scale was used to evaluate and record the results of all subjects who were followed up to the corrected gestational age of 40±2week.The differences in hearing screening,head imaging,brain injury markers NSE,S100B protein and NNNS neurobehavioral assessment between the two groups were compared and analyzed.SPSS22.0 statistical software was used for processing.The quantitative data were expressed as mean±standard,the quantitative data were compared by student’s t test,the qualitative data by rate,and the qualitative data by chi-square test.P<0.05 was considered statistically significant.Results:(1)、There was no significant difference in the rate of the failure of initial hearing screening(30%)between preterm infants in the study group and preterm infants in the control group(17.5%)(P>0.05),nor was there significant difference in the rate of the failure of the review of hearing screening(12.5%)between preterm infants in the study group and preterm infants in the control group(5%)(P>0.05);(2)、The abnormality rate of the head imaging of premature infants in the study group(22.5%)was not significantly different from that of the control group(5%)(P>0.05).The NSE[(36.76±13.37)ng/ml]of premature infants in the study group was higher than that in the control group[(20.37±9.72)ng/ml],and the difference between the two groups was statistically significant(P<0.001).The S100B protein[(0.65±0.15)ug/l]of premature infants in the study group was higher than that in the control group[(0.27±0.13)ug/l],and the difference between the two groups was statistically significant(P<0.001);(3)、Comparison of NNNS neurobehavioral assessment between two groups:the three differences between the two groups in 13 assessment programmes are habituation,excitability,and hypotonicity.The scores of the study group premature habituation(3.99±0.55)was lower than those of control group(5.74±0.63),the difference was statistically significant(P<0.05);The scores of the study group premature excitability(3.93±0.18)higher than the control group(1.58±0.81),the difference is statistically significant(P<0.05);The scores of the study group preterm hypotonicity(0.15±0.36)higher than the control group(0.02±0.15),the difference is statistically significant(P<0.05).Conclusion:(1)、Transient hypothyroidism of premature infants is not significantly correlated with hearing impairment;(2)、The biological brain injury markers NSE and S100B protein of THOP in premature infants are relatively high,and transient hypothyroxemia is a high risk factor of brain injury in premature infants;(3)、Premature infants with THOP are more likely to be irritated in the early stage,and have poor regulation of external environmental stimuli,which have some influence on the early maturity and development of the nervous system.It can provide guidance for reducing the incidence of long-term neurodevelopmental disorders in premature infants.
