Objective To investigate the relationship between the incidence of transient hypothyrodism in preterm infants and gestational age, birth weight and disease, and to investigate its clinical manifestations. Methods Electrochemistry luminescence was used to detect the blood thyroxine (T4), free thyroxine (FT4), three of triiodothyronine (T3), free three triiodothyronine (FT3), thyroid stimulating hormone (TSH) of196preterm infants on the7th day. We analysis the relationship between the incidence of transient hypothyrodism and gestational age, birth weight and disease, and, on the14th day, investigate the number of cases which have restored to birth weight, the number of cases which have regular bowel movements, the severity of jaundice.Results1. The rate of TH-positive is35.7%(70/196).2. There is no difference between male and female in the rate of TH-positive in preterm infants (p>0.05).There is difference between very low birth weight group and control group (p<0.05).There is difference between small gestational age group and large gestational age group (p<0.01).There is difference between HIE group and non-HIE group (p<0.01).There is difference between NRDS group and non-NRDS group (p<0.01). There is difference between infection group and non-infection group (p<0.01).There is difference between several illnesses group and control group (p<0.01)3. The gestational age of the TH group is smaller than the non-TH group(p<0.01). The birth weight of the TH group is smaller than the non-TH group(p<0.01).4. On the14th day,the rate of restored to birth weight case in the TH group is smaller than the non-TH group(p<0.005) and the rate of having regular bowel movements in the TH group is smaller than the non-TH group(p<0.005). But there is no difference in the rate of jaundice between TH group and non-TH group (p>0.05)Conclusion The rate of TH-positive in this group is35.7%. The occurrence of transient hypothyrodism in preterm infants is related to gestational age, birth weight. The incidence is higher in preterm infant suffering from the disease (HIE, NIDS, infection, several illnesses).
