Backgound: A large number of laparoscopic pancreatoduodenectomy and openpancreatoduodenectomy have been carried out in Europe and the United States to treat Pancreatic cancer and Periampullary cancer.However,LPD technology for carcinoma of the papilla of Vater is still lacking the long-term overall survival rate.Objective:To analyze the long-term efficacy of laparoscopic pancreaticoduodenectomy for patients with Carcinoma of the papilla of the Vater.Methods: Clinical data of 89 patients with Carcinoma of the papilla of the Vater from January 2012 to December 2017 were retrospectively analyzed.Results: The operation time of LPD was shortened [(285.3±35.3)min vs(315.7±47.2)min,P=0.860],and the intraoperative blood loss was significantly reduced[(308.0±186.8)ml vs(163.0±134.3)ml,P=0.021].The postoperative hospital stay was shortened [(23.7±15.5)d vs(15.8±2.3)d,P=0.043],intraoperative lymph node clearance number[(16.8±5.74)vs(18.9±3.65),P=0.386],positive lymph node number[(0.74±1.5)vs(1.12±2.72),P=0.117],the R0 resection rate was 96.0% vs97.4%,P=0.593.The postoperative Clavien grade?III grade complications were 4 and 4,P=0.726.There was no significant difference in 1 year,3-year survival rate(86.8% vs.87.2%,43.6% vs.46.2%,P=0.686).Conclusion: Laparoscopic pancreatoduodenectomy has the same R0 resection and lymph node harvested as open pancreatoduodenectomy in patients with carcinoma of the papilla of Vater.The long-term overall survival rate are equivalent to open pancreatoduodenectomy.
