| Objective: To investigate the effects of drugs(antacids,depolymerizing drugs,lipid-lowering drugs,anticoagulants)on the incidence of acute kidney disease in hospitalized elderly patients.Methods: Clinical data of elderly patients aged 60 years and older who were admitted to the Hunan Provincial People’s Hospital from January 1,2012 to September 30,2018.The age and medication status of the patients,the changes of serum creatinine and glomerular filtration rate before and after treatment were analyzed,and the data were statistically analyzed.RESULTS: A total of 86,141 elderly patients were hospitalized during the same period.A total of 1246(1%)patients with renal function and two or more were included in the study.Among them,408 cases were not used any durg,and 128 cases(31.37%)of AKD occurred.There were 838 cases in the drug group,369 cases(44.07%)of AKD occurred,of which 54 cases were statin and 25 cases(46.30%)were AKD.There were 55 cases of antiplatelets and 20 cases of AKD(36.36%).There were203 cases of proton pump inhibitor and 103 cases(50.74%)of AKD.A total of 294 cases of statin and antiplatelet were combined,and 113 cases(38.44%)of AKD occurred.There were 21 cases of statin and proton pump inhibitor,and 13 cases(61.9%)of AKD occurred.In 12 patientswith antiplatelet and proton pump inhibitors,no AKD(0%)occurred.In55 patients with statin,antiplatelet and anticoagulant,37 patients(67.27%)developed AKD.In combination with antiplatelet,statin and proton pump inhibitors in 79 patients,23 patients(29.11%)developed AKD.Combined with statin,antiplatelet,anticoagulant,proton pump inhibitor in 65 cases,35 cases of AKD occurred(53.85%).Logistic regression analysis showed that hospitalization with proton pump inhibitors,combined use of statins and proton pump inhibitors,and age increased the risk of AKD.CONCLUSION:(1)AKD is more likely to occur in elderly people aged 75 years and older.It is recommended that patients who are hospitalized for more than one week at age 60 and above undergo at least two renal function tests.(2)Use antiplatelet or proton pump inhibitor,combination of statins and proton pump inhibitors,combination of statins,antiplatelet and anticoagulants may increase the incidence of AKD for those 60 years of age and older.(3)Combination of statins and antiplatelet drugs in hospitalized patients aged 75 years and older,and combination of statins,antiplatelet and proton pump inhibitors did not show a higher risk of AKD. |
