| Objectives(1)To understand the patients with stage 5 chronic kidney disease ’s nutritional status,health literacy,anxiety/depression status;(2)To analyze the influencing factors of nutritional status of patients with stage 5 chronic kidney disease;(3)To explore the correlation between nutritional status and health literacy,anxiety/depression of patients with stage 5 chronic kidney disease.MethodsThis study uses random sampling method.From May 2018 to September 2018,patients with stage 5 chronic kidney disease who met the inclusion and exclusion criteria were randomly selected from three tertiary first-class hospitals in Changsha city for questionnaire collection.Patients with odd numbers at the end of hospitalization numbers were selected on odd days,and patients with even numbers at the end of hospitalization numbers were selected on even days.They were investigated with general data questionnaire,subjective comprehensive nutrition assessment scale(SGA),health literacy scale and comprehensive hospital anxiety/depression rating scale(HADS).All data were entered and analyzed by SPSS20.0 software.Descriptive analysis,T-test,Chi-square test,logistic regression and Spearman correlation analysis were adopted.Results(1)In this study,134(67.0%)of patients with stage 5 chronic kidney disease were well nourished and 66(33%)were malnourished.Of all the items,only the item of activity ability change has the largest number of B(moderate decrease or improvement in strength/energy),accounting for 45.5% of the total number.(2)The average scores of anxiety and depression in patients with stage 5 chronic kidney disease were 6.45±3.58 and 7.47±3.79 respectively.40.5% patients had anxiety symptoms of different degrees.43.0% patients had depressive symptoms of different degrees.(3)Of the 200 patients,only 49% HADS health literacy and 51% lacked health literacy.The total score of health literacy was 93.91±16.59.The patients with stage 5 chronic kidney disease were generally lacking in health literacy.Compared with the domestic norm,the scores of all dimensions of health literacy,information acquisition ability,willingness to improve health and willingness to support economy were lower than the domestic norm,and only the dimension of communication and interaction ability was higher than the norm,the difference was statistically significant(P<0.05).(4)Univariate analysis showed that there were differences in nutritional status of patients with stage 5 chronic kidney disease in terms of age,place of residence,occupational status,educational level,family per capita monthly income,medical expenses payment method and self-care ability(P<0.05).Logistic regression analysis results showed that the family’s average monthly income,health literacy and anxiety/depression entered the regression equation.(5)Spearman correlation analysis showed that the correlation coefficients of anxiety,depression and nutritional status were 0.292 and 0.408 respectively,showing positive correlation(P<0.01).The correlation coefficient between health literacy and nutritional status was-0.335;The correlation coefficient between information acquisition capability dimension and nutritional status was-0.351;The correlation coefficient between communication and interaction ability dimension and nutritional status was-0.280;The correlation coefficient between the dimension of willingness to improve health and nutritional status was-0.265,which was negatively correlated(P<0.01).Conclusion(1)33% stage 5 chronic kidney disease patients suffer from malnutrition.Age,residence,education level,occupation status,average monthly household income,medical expenses payment method and selfcare ability are the influencing factors.(2)The level of health literacy of patients with stage 5 chronic kidney disease is generally low.51% of patients lack health literacy;The incidence rate of anxiety/depression in patients with stage 5 chronic kidney disease is relatively high,accounting for 40.5% and 43.0% respectively.(3)There is a positive correlation between anxiety,depression and nutritional status in patients with stage 5 chronic kidney disease,that is,the more serious the anxiety/depression,the worse the nutritional status;The nutritional status of patients with stage 5 chronic kidney disease is negatively correlated with the level of health literacy and its dimensions of information acquisition capability,communication interaction capability and willingness to improve health,that is to say,the higher the level of health literacy of patients,the better the information acquisition capability and communication interaction capability,the stronger the willingness to improve health,and the better the nutritional status. |
