| Objective Glucose tolerance reduction is the pre-diabetes stage and an important disease process to prevent the occurrence of diabetes.Therefore,effective counterm easures can prevent the occurrence of diabetes.In this study,patients with impaired glucose tolerance and hyperlipidemia were selected as subjects to study their adiponectin levels and the changes after the intervention of metformin,so as to master the relevant mechani sms of clinical disease development and provide theoretical basis for the early intervention off impaired glucose tolerance and the developm ent and clinical application of adiponectin and its analogues.Methods From September 2014 to May 2016,53 patients with simple IGT in our hospital were selected as the IGT control group in the high-risk population of diabetes,including 31 males and 22 females,aged 40 to 65 years old,with an average age of(53.1±1.4)years.54 patients with IGT complicated with hyperlipidemia were selected as the case group,including 33 male patients and 21 female patients.The age range was 42-63 years old,with an average age of 53.0±1.2 years.Age,gender,blood pressure,body mass index(BMI),waist to hip ratio(WHR)and other indicators were collected.Body mass index(BMI)= weight(kg)/height(m2);Blood pressure,waist-hip ratio(WHR)=waist circumference(cm)/hip circumference(cm)were measured using the right brachial artery.Venous blood was extracted on an empty stomach in the morning for blood glucose,blood lipids,insulin,c-peptide and adiponectin.After oral administration of150 ml 50% glucose and 100 ml water,blood samples were taken at 120 minutes to measure c-peptide,blood lipids and blood glucose.Patients with impaired glucose tolerance and hyperlipidemia were selected to take metformin 0.5g orally 3 times a day.Six months later,venous blood was drawn on an empty stomach in the morning for blood glucose,blood fat,insulin,c-peptide,adiponectin,etc.To observe the changes of glucose and lipid metabolism indexes and adiponectin levels in IGT patients with hyperlipidemia before and after metformin treatment.All the study data were included in SPSS20.0 software,and Logistic regression analysis was used to analyze the related factors affecting adiponectin level,and the test standard P<0.05.Result(1)Univariate analysis showed that the level of adiponectin in the case group was significantly lower than that in the control group,and the difference between groups was statistically significant(P <0.05),the level of adiponectin was decreased,and the risk of IGT combined with hyperlipidemia was increased(P <0.05).(2)Multivariate Logistic regression analysis showed that after the influence of confounding factors was excluded,reduced adiponectin level was a risk factor for IGT combined with hyperlipidemia.(3)After 6 months of metformin treatment,the improvement of blood glucose,lipid,insulin,adiponectin,TC,TG,HDL-C and BMI of the patients was greater than that of the diet and exercise group,and the difference was statistically significant(P<0.05).Conclusions The serum adiponectin level of IGT patients with hyperlipidemia was decreased,and metformin could effectively improve the serum adiponectin level of the patients.Figure 0;Table 16;Reference 133... |
