Objectives To explore the clinical effect of noninvasive ventilator on patients after lobectomy,and further analyze the effect of noninvasive positive pressure ventilation(NPPV)on postoperative pulmonary function.Methods Eighty patients in our hospital from November 2017 to October 2018 and who underwent lobectomy during hospitalization were enrolled.The patients were divided into treatment groups(40 cases)and control group(40 cases)according to whether they used non-invasive ventilators after surgery.The control group received routine postoperative treatment(oxygen,anti-infection,analgesia,hemostasis and other symptomatic supportive treatment),and the treatment group was treated with NPPV for 8 hours on the basis of the control group.The arterial blood gas analysis(Pa O2,Pa CO2)index at three time points before noninvasive ventilation(T1),noninvasive ventilation 1 hour(T2)and noninvasive ventilation 8 hours(T3)were performed.Preoperative,postoperative 1 month and postoperative 3 months pulmonary function parameters at three time points [percentage of forced expiratory volume in 1st second(FEV1%),percentage of lung diffusion function(DLCO%)],thoracic drainage,drainage time,KPS score and hospitalization time,etc,statistical analysis was performed on these data.The above clinical factors were used to evaluate the difference in clinical outcomes between non-invasive ventilator therapy and conventional therapy.Results 1 Compared with the control group,the difference of PaO2 and PaCO2 in the treatment group at the T2 and T3 time points were statistically significant(P<0.05);the difference of FEV1% and DLCO% at the first month postoperative time were statistical significance(P<0.05),and there was no statistically significant difference at the time point of the third month after operation(P>0.05).2 Analysis of variance of repeated measurements of Pa O2 and Pa CO2 in two groups of patients,Pa O2 interaction test with different time points and groups Partial Eta Squared=0.696,F=87.937,P=0.000<0.05,indicating that the trend of Pa O2 in different groups with time points is significantly different,it is statistically significant;Pa CO2 interaction test with different time points and groups Partial Eta Squared=0.943,F=631.268,P=0.000<0.05,indicating that the trend of Pa CO2 in different groups with time points is significantly different,it is statistically significant.3 There were significant differences in thoracic drainage,drainage time,hospitalization time and postoperative KPS score between the treatment group and the control group(P<0.05).Conclusions 1 Non-invasive ventilator used in patients after pneumonectomy can improve the respiratory function of patients to a certain extent,and has a good auxiliary effect for early recovery after surgery;2 Postoperative application of NPPV can promote the recovery of early lung function in patients.To some extent,the patient's postoperative recovery is accelerated.Figure 7;Table 10;Reference 142...
