Objectives To quantitatively assess the difference of pulse wave velocity with ufPWV in ISH,SDH,hypertensive blood pressure control group and healthy control group,so that evaluate the carotid artery elasticity of these four group,and evaluate the vascular differences between the elderly population with isolated systolic hypertension and other groups;To evaluate the potential value of the two parameters detected by ufPWV included BS and ES in the early damage of atherosclerosis and the risk of cardiovascular and cerebrovascular diseases.Methods 209 subjects from December 2017 to September 2018 in North China University of Science and Technology Affiliated Hospital were studied: 59 patients with ISH(SBP?140mmHg and DBP<90mmHg)(mean age 59.03±6.71 years),44 aged-matched SDH(SBP?140mmHg and DBP?90mmHg)(mean age 55.60±7.76 years),55 patients with hypertensive blood pressure control group(mean age 58.24±7.31 years),and 55 agedmatched healthy control subjects(mean age 56.38±8.60 years).The diameter,IMT and spectrum parameters of CCA was measured with traditional method.The ultrafast PWVs at the beginning and ending of systole were automatically measured.The differences of the above parameters between the four groups were analyzed and compared.Gender,age,heart rate and blood sugar were matched among the four groups,excluding the influence of confounding factors.Futhermore,family history,medication history,target organ damage and complications were recorded.Database was established by Excel,and statistical analysis were performed on SPSS 24.0.One-way ANOVA was used to compare measurement data,and the use LSD test to compare component differences.Pearson chi-square was used to compare Enumeration data.Rank sum test was used to compare measurement data;A value of P 0.05 was considered significant.Results SBP: SDH,ISH group>control normal group>control group;DBP : SDH group> ISH group>control normal group and control group;PP: ISH group>SDH group,control normal group>control group;Patients with control group have lower CCA diameter and IMT compared with those three essential hypertension group,but there was no significant difference among the latter three group.That is to say,these two traditional indexes for evaluating atherosclerosis could only be differentiated between the control group and hypertension group,however,there were diagnostic limitations for different types of blood pressure group.BS: ISH group>SDH group,control normal group>control group;ES: ISH group>SDH group,control normal group>control group,then the patients with ISH has the highest PWV;ISH have higher risk of arteriosclerosis compared with SDH.UFPWV is more sensitive than IMT in early evaluation of atherosclerosis.The group that patients with higher PWV had higher probability of target organ damage and cardio-cerebrovascular events compared with lower PWV,that is to say,PWV has potential predictive value for the occurrence of cardiovascular and cerebrovascular events.Conclusions 1 ufPWV is an effective and non-invasive method for evaluating early vascular elasticity changes.ISH patients compared with control group,SDH patients and hypertension blood pressure control people have higher BS and ES values and lower vascular elasticity.2 Compared with IMT,PWV can detect the elastic changes of vascular wall earlier.3 Patients with higher PWV(including BS and ES)have higher risk of cardiovascular and cerebrovascular events compared with Patients with lower PWV and predict the occurrence of cardiovascular events.Figure2;Table12;Reference 80...
