| Object: To analyze the the current situation and influencing factors of home-based informal care time for disabled elderly in Xinjiang pastoral area and further explore the ways of key influencing factors on the informal care time.The home-based informal care time cost was calculated in order to provide scientific decision-making basis for the government departments concerned to compensate the investment of informal care in Xinjiang pastoral areas.Methods: Using quantitative and qualitative research methods to analyze and calculate home-based informal care time cost for the Kazakh disabled elderly in Xinjiang pastoral area.A field survey was conducted among 352 dyads of Kazakh disabled elders and their primary informal caregivers selected from 1204 elderly kazak people in pastoral areas by a multi-stage stratified random cluster sampling method.The home-based informal care time cost was accounted for by opportunity cost method and proxy good method.Semistructured interviews were conducted with people closely related to home-based informal care,including caregivers and local civil affairs bureau,finance bureau,health bureau,aging office and other social security personnel.SPSS23.0 was used for data description,independent-samples t test,one-way anova,rank sum test,multiple linear regression analysis,correlation analysis and hierarchical regression analysis.Results: 1.The total home-based informal care time for the Kazakh disabled elderly in Xinjiang pastoral area is 5.72±2.108 h/ day/person,higher than the results of other domestic schola.Among them,the total homebased informal care time for the elderly with mild,moderate and severe disability is 4.55±0.719 h/ day/person,6.87±0.685 h/ day/person,10.08±1.276 h/ day/person respectively.The disability severity of elders,home-based informal caregivers' gender,age,care ability,health status,whether the children of caregivers need to be cared,social support and care burden are the influencing factors of informal care time,explaining 94.4% of the total variation.The disability severity of elders(Beta=0.847)and the health status of the caregivers(Beta=0.135)have the greatest influence on the home-based informal care time2.The disability severity of elders,health status of home-based informal caregiver,and informal care time are correlated with each other.The standardized regression coefficient of the interaction is 0.067,P<0.05,besides,?R2 = 0.004,P<0.05,explaining that the health status of informal caregivers has moderating effect in the relationship between the disability severity of elders and informal care time.The mediating effect of caregivers' health status between the disability severity of elders and informal care time is 10.01%.It shows that disability severity of elders mainly has a direct impact on the informal care time.3.According to opportunity cost method and proxy good method,the home-based informal care time cost of Kazakh disabled elderly in Xinjiang pastoral area is 1132.56 RMB/month/person and 952.38 RMB/month/person respectively,which is far lower than those in eastern,central and western China.Conclusions: 1.The home-based informal care time of the Kazakh disabled elderly in Xinjiang pastoral areas is generally higher,and the disability severity of elders and the health status of home-based informal caregivers havea greater impact on the informal care time.The health status of caregivers can influence the relationship between informal care time and the disability severity of elders.The disability severity of elders mainly have a direct impact on the home-based informal care time.2.Home-based informal care time cost of the Kazakh disabled elderly in Xinjiang pastoral areas is lower than those of eastern,central,western China.Reasonable home-based informal care time cost compensation policy should be given priority to the elderly with poor disability and the families with poor health status of the caregiver,adapt to local conditions,and combine the local government finances to make economic compensation. |
