Research On The Status Of Clinical Application Management Of Medical Technology

Objective: In 2015,the State Council issued the “Decision on Cancellation of Non-Administrative License Approval Items”,which changed the access approval system for the clinical application of the second and third types of medical technology.In the context of the implementation of the new medical technology management model in Chongqing in August 2015,this paper investigates the clinical application of Chongqing medical technology through the investigation of the status of technical projects,access approval and filing publicity of medical institutions in Chongqing.At the end,analyze the problems in the clinical application management of medical technology,find the bottleneck in the implementation process and put forward corresponding suggestions,aiming at strengthening the supervision of the clinical application of medical technology and improving the implementation effect of the New Deal.Method: Combine quantitative and qualitative research by reviewing relevant research literature and on-site form surveys to collect relevant data.On-site investigations were conducted on 198 medical institutions involved in the development of Class II and restricted medical technologies in 16 districts(counties)in Chongqing using a stratified and multi-stage comprehensive survey.Statistical analysis and evaluation of quantitative data were performed using the method of health statistics.Descriptive analysis method was used to analyze the basic situation of medical institutions and the review and record of clinical application of medical technology.Using the chi-square(two-sided)analysis method to compare the approval and filing of clinical application of medical technology in different regions and levels,the indications and publicity of medical technology in different regions,and the approval filing rate and the publicity rate before and after the new management mode.Results: In this survey,a total of 198 medical institutions included in the survey were issued paper and electronic forms,and 198 copies of valid paper and electronic forms were collected.Most of the respondents were social organizations,including 112,accounting for 56.57%,86 public medical institutions,32 tertiary medical institutions,50 secondary medical institutions,and 4 primary medical institutions.As of December 31,2017,a total of 142 medical institutions in Chongqing had carried out the second category of medical technology,and 19 medical institutions carried out restricted medical technology.All the medical technologies that have been carried out involved 47 types and 849 items.Through the analysis of the approval of the second type of medical technology access,it is found that the differences in medical technology audit rates of medical institutions of different natures are statistically significant;the differences in the audit rates of medical institutions with different jurisdictions are not statistically significant;the difference in the rate of medical technology between the two was not statistically significant.The analysis of the disclosure of the second type of medical technology shows that the difference in the medical technology filing rate of medical institutions of different natures is statistically significant;the difference in medical technology filings between jurisdictions is statistically significant;the difference in publicity rates across jurisdictions was not statistically significant.A comparison of the management effects before and after the implementation of the new management measures found that the difference in the completion rate of administrative procedures between medical institutions around August 2015 was statistically significant(P<0.01);the difference in the completion of the procedures of the health and family planning administrative department did not statistical significance(P=0.558>0.05).The survey of attitudes of medical institutions on the new policy of clinical application management of medical technology shows that the regulators of the policy have a high sense of recognition for the new mode of medical technology management,and the response performance is outstanding.Conclusion: After adjusting the regulatory procedures for clinical application of medical technology in 2015,from the current situation of clinical application management of medical technology,the introduction of medical technology under the new management mode is strong,and medical institutions are more responsive to the new management model.However,the impact on the completion rate of the management procedures of medical institutions and health administrative departments was not significant,and the completion rate did not increase as expected.It is necessary to strengthen the publicity of the medical technology management system,improve the awareness of active management of medical institutions,improve the efficiency of the administrative department,update the second-class medical technology catalogue,and strengthen the tracking and management of medical technology.Starting from the two aspects of the supervisory body and the regulated subject,we will comprehensively improve the supervision and management effect of medical technology,ensure the effective implementation of the medical technology supervision and management system and the efficient implementation of the corresponding management work.
