| Objective:To explore the clinical characteristics of neonatal appendicitis(NA)and reduce the misdiagnosis as well as improve clinical outcome.Methods:The data were collected from acute appendicitis patients under 1 year old who were admitted to our hospital from February 1993 to July 2018.Clinical characteristics of patients from neonatal group and non-neonatal group were compared and analyzed.Results:There were 87 cases,including 31 newborns and 56 non-n ewborns.The two groups have statistical differences in the incidence of sex,perforation,pathological type,abdominal distension,vomiting,milk refusal and weak cry,irritable crying,abdominal distension,exposure of abdominal vein,abdominal wall redness and swelling,right inguinal ma ss/abdominal mass,and decreased bowel sound(P<0.05),but there was no significant difference between the two groups in the incidence of fe ver,blood stool,tender,voluntary guarding(P>0.05).The proportion ofperitonitis,intestinal obstruction and sepsis in neonatal group was signi ficantly higher than that in non-neonatal group,and only the incidence of sepsis has significant difference(P<0.05).In the neonatal group,ther e were 10 cases combined with NEC,8 inguinal hernia,the incidence o f NEC has significant difference between the two groups(P<0.05).The re was no significant difference in abdominal ultrasound and prognosis between the two groups(P>0.05).Conclusion:Neonatal appendicitis has long hospital stay,perforates easily and lacks specific symptoms and signs,so when the children have abdominal distention and other gastrointestinal symptoms,and abdominal signs indicate inflammation in the abdominal cavity,we should be on high alert.Therefore,diagnosis should be made after combining clinical manifestations,abdominal ultrasound and exploratory laparotomy when necessary,to treat earlier and improve the prognosis.The relationship between neonatal appendicitis and HD,gastroenteritis and so on is not completely clear,and it is worth further study. |
