Objective:To understand the current status of antenatal risk of postpartum depression among pregnant women in Chengde,and to analyze the influencing factors of antenatal risk and its correlation with perceived stress and social support,so as to provide a basis for early intervention measures for pregnant women with antenatal risk in the future.Methods:1450 pregnant women was selected by the successive sampling method.From March to December 2018,pregnant women in the second or third trimester(?24 weeks of gestation)who participated in the pregnancy examination were selected as subjects in two first-class hospitals in Chengde,Antenatal Risk Questionnaire-Chinese,Social Support Rating Scale,Chinese Perceived Stress Scale and general situation questionnaire(self-made)were used for evaluation,SPSS 22.0 was used to analyze the survey data.Analytical methods included descriptive statistical analysis,chi-square test(two-sided test ?=0.05),and binary unconditional logistical regression analysis(?_in=0.05,?_out=0.10)etc.Results:1.In this survey,a total of 1560 questionnaires were distributed and 1450 questionnaires were effectively recovered,with an effective recovery rate of 92.9%.The antenatal risk of postpartum depression was 13.7%.2.Univariate analysis of factors affecting antenatal risk of postpartum depression: Chi-square test showed that there were statistically significant differences between the high-risk group and the low-risk group(P <0.05):Different ages,education level,production time,working conditions during pregnancy,feelings after pregnancy,degree of concern of husband,support and concern from family members,whether there was concerns about family economy,whether there was depressed emotion lasting for more than two weeks during pregnancy,whether there were anxious or irritable emotion lasting for more than two weeks during pregnancy,Whether the relationship between mother-in-law and daughter-in-law is harmonious,Whether the relationship between husband and wife is harmonious,etc.3.Correlation analysis between maternal social support or perceived stress and prenatal risk of postpartum depression: Spearman correlation analysis showed that social support is negatively correlated with antenatal risk of postpartum depression(r_s=-0.329,P < 0.001),and perceived stress is positively correlated with prenatal risk of postpartum depression(r_s=0.382,P<0.001).4.Multiple analysis of factors influencing antenatal risk of postpartum depression: By binary logistic regression analysis,including the age(OR=1.556,95%CI:1.168~2.072),working conditions during pregnancy [ "home"(OR=0.471,95%CI:0.298~0.745),"full-time"(OR=0.283,95%CI: 0.176~0.456)],parity(OR=2.182,95%CI:1.404~3.391),whether there was a depressed emotion more than two weeks during pregnancy(OR=1.720,95%CI:1.148~2.577),whether there was anxiety and irritability for more than two weeks during pregnancy(OR=1.681,95%CI:1.104~2.558),relationship with mother-in-law(OR=2.736,95%CI:1.988~3.766),help and concern from family members(OR=1.980,95%CI:1.497~2.619),perceived stress levels(OR=3.797,95%CI:2.535~5.686)and social support level(OR=0.606,95%CI:0.495~0.741),are related to the occurrence of antenatal risk.Conclusion:1.The antenatal risk detection rate of postpartum depression in Chengde was 13.7%.2.Social support is negatively correlated with antenatal risk of postpartum depression;and perceived stress is positively correlated with prenatal risk of postpartum depression.3.The influencing factors antenatal risk of postpartum depression include: age,working conditions during pregnancy,production time,whether there are depressed emotion that last for more than two weeks during pregnancy,whether there are anxiety and irritability that lasts for more than two weeks during pregnancy,relationship with mother-in-law,help and care from family members,perceived stress levels,social support level.Therefore,early prevention of postpartum depression should be started from pregnant women with high antenatal risk.Early interventions for the influencing factors be benefit to prevent or reduce postpartum depression and promote physical and mental health of pregnant women.
