Objective:To investigate the incidence of complications and risk factors of second-child pregnant women after the implementation of the comprehensive two-child policy in order to reduce the incidence of adverse pregnancy outcomes.Methods:1 Data of pregnant women who delivered in the Affiliated Hospital of Chengde Medical College before the implementation of the "two-child open policy" from November 2013 to May 2014 and one year after the implementation of the "two-child open policy" from November 2016 to May 2017 were collected.The data were divided into two groups,A and B,respectively.The incidence of complications during pregnancy of two groups of pregnant women was compared by test.2 467 second-child pregnant women in group B were divided into two groups.217 cases with complications during pregnancy were divided into case group and 250 cases without complications during pregnancy were divided into control group.Age,education level,pre-pregnancy BMI,post-pregnancy abortion history,family history,bad pregnancy history,interval time between one fetus,number of obstetric examinations,weight gain during pregnancy,number of pregnancies and nutrition guidance during pregnancy were given to the two groups.Factor test and multivariate unconditional logistic regression analysis.Results:1 Comparing group A with group B,the delivery volume of group B increased significantly,and the incidence of complications in group B was higher than that in group A.The difference was statistically significant.(P < 0.05).2 Univariate analysis results showed that there were significant differences between the case group and the control group in terms of the advanced age,low educational background,increased BMI before pregnancy,slight or excessive weight gain during pregnancy,more than three pregnancies,family history of disease or adverse pregnancy,long interval with one fetus and non-nutritional guidance during pregnancy(P<0.05).Multivariate conditional logistic regression analysis showed that increased BMI before pregnancy(OR=2.655,95%CI:1.761~4.004),long interval with one fetus(OR=1.420,95%CI: 1.013~1.992),family history of disease(OR=1.829,95%CI:1.054~3.174)or adverse pregnancy(OR=20.969,95%CI:4.340~101.309),more than three pregnancies(OR=1.601,95%CI:1.018~2.519)and non-nutritional guidance during pregnancy(OR =0.346,95%CI:0.220~0.543)are risk factors for complications of second-child pregnancy.Conclusion:1 After the implementation of the comprehensive "two-child open policy",the total delivery volume and the incidence of complications during pregnancy of second-child pregnant women increased significantly.2 Pre-pregnancy BMI,interval time with one fetus,family history of disease or adverse pregnancy,number of pregnancies and nutritional guidance during pregnancy are associated with complications of second-child pregnancy.
