Research On The Effects And Constrictions Of Loosely Affiliated Hospital Alliance

China is now facing the problem of aging acceleration and the increasing rate of chronic disease.Furthermore,China's economy has entered a period of new normal.Seeing a doctor randomly is not only harmful to patients,but also cause a huge waste of medical resources.As an important method in hierarchical diagnosis,hospital alliance is developing rapidly throughout the country.What has loosely affiliated hospital alliance which is the main integration form at this stage actually achieved? What are the problems? What are the factors that restrict its development?In this paper,based on the " Structure-Process-Outcome" theory,the evaluation index system of the effect of loosely affiliated hospital alliance was established.Then using stakeholder theory,the key constraints were explored.Through two cases and in-depth interviews with several medical workers,the study found that:(1)Loosely affiliated hospital alliance has strengthened communications among medical institutions to some extent,but forms surpass contents.Hospitals at all levels lacks impetus.(2)Lack of interest distribution mechanism,improper government management and lack of supporting policies are three key constraints.Based on the above analysis,this paper argues that loosely affiliated hospital alliance remains in the stage of cooperation with each other.The high quality resources have not yet been able to circulate and the service capabilities of primary medical institutions are not improved.As the main promoter of hospital alliance,policy making should lay emphasis on these two aspects,such as establishing a unified management system in the hospital alliance,promoting informatization and ensuring the sharing of high quality resources through an incentive system;the government needs to increase support to community health centers so as to improve their service level and the trust of residents.At the same time,it is necessary to increase the publicity of hospital alliance to stakeholders.Only by this way can loosely affiliated hospital alliance play its real function.
