Objective :to evaluate the efficacy and safety of traditional Chinese medicine(TCM)in the treatment of chronic kidney disease(CKD)by observing its intervention based on the theory of combining disease and syndrome with treatment.so as to provide evidence-based medical evidence in clinical trials for the treatment of CKD and provide new ideas for the follow-up treatment of CKD.Methods : a total of 48 patients with chronic kidney disease who met the diagnostic of chronic kidney disease and the inclusion and exclusion criteria were selected and admitted and admitted to the outpatient clinic of professor li mingquan, department of nephrology, affiliated hospital of chengdu university of traditional Chinese medicine/sichuan hospital of traditional Chinese. On the basis of treatment of western medicine,TCM treatment plan of diseases and syndromes(TCM decoction, one dose a day, three times a day,100 ml each time, take warm for half an hour after meals)was given. After 3 months of treatment and follow-up,the clinical symptoms,TCM syndrome scores, serum creatinine(Scr),urea nitrogen(BUN),estimated filtration rate(eGFR)and other relevant therapeutic indicators were observed before and after treatment. As well as blood routine, liver function and other safety indicators.Through data and data collation,SPSS19.0 statistical software was used for statistical comparison and analysis.Results:1.Curative effect: 6 cases were effective,12 cases were effective,18 cases were stable,12 cases were ineffective, the total effective rate was 75%.2.Curative effect of TCM syndromes: 6 cases were cured,15 cases were obviously effective,22 cases were effective,5 cases were ineffective, and the total effective rate was89.58%.3.TCM syndrome score: the total score of TCM syndrome after treatment was significantly lower than that before treatment, with P value < 0.01,and the difference was statistically significant.4.Clinical symptoms of traditional Chinese medicine: after treatment, all clinical symptoms of traditional Chinese medicine were significantly improved compared with before treatment,P < 0.05,and the difference was statistically significant.5.Laboratory indexes: the glomerular filtration rate was higher than before treatment, and the difference was statistically significant(P < 0.05).Creatinine was lower than before treatment, and P < 0.05,the difference was statistically significant.Urea nitrogen was lower than before treatment, but P > 0.05,the difference was not statistically significant.6.Safety index: red blood cell, hemoglobin, alanine aminotransferase, aspartate aminotransferase, before and after treatment,P > 0.05,no statistical difference. The difference was statistically significant(P < 0.05).Conclusion:1.The combination of disease and syndrome therapy for chronic kidney disease can significantly alleviate the clinical symptoms of patients with chronic kidney disease, and has the effect of reducing blood creatinine, increasing glomerular filtration rate.To some extent,it can control and delay the progress of renal function.2.Through the observation of the safety indexes, there was no abnormal blood routine and liver function during the clinical observation.During the observation, serum potassium increased in 2 cases, but all < 6.0mmol/L.Among them,1 patient was instructed to pay attention to the improvement of indicators after diet, and 1patient was instructed to improve indicators after the addition of furosemide. This TCM treatment scheme is not only effective, but also safe. The overall level of potassium after treatment was higher than that before treatment, indicating that traditional Chinese medicine can cause fluctuations of serum potassium. In addition, there were no adverse clinical reactions such as nausea, vomiting, diarrhea and skin allergy. Therefore, electrolytes should be monitored dynamically during medication.
