Objective:To investigate the clinical application of preoperative transanal drainage catheter in the treatment of low and operable malignent intestinal obstruction.Methods:82 patients with colorectal cancer obstruction admitted to our hospital from June 1,2010 to June 30,2018 were divided into two groups according to different treatment methods.In the treatment group,43 patients underwent one-stage resection and anastomosis after decompression,irrigation and drainage,intestinal preparation.There were 23 males and 20 females,aged 35-76,43cases of left colon and rectum obstruction.Forty-three ases were successfully placed.After successful catheterization,43 recruited patients continued decompression,cleaned the intestine,completely relieved obstruction and could eat normally.One week later,they were transferred to surgery for one-stage resection of tumors.There were no complications such as hemorrhage and perforation of digestive tract in 43 cases.39 cases in the traditional treatment group were treated with the traditional treatment method,and no transanal drainage catheter was used.There were 21 males and18females,aged 38-80,The remission rate of early obstruction,the success rate of stage I operation,hospitalization time,hospitalization expenses and the incidence of complications were observed.The difference between the two groups was compared by means of t test and?~2 test.Results:The one-day remission rate of symptoms in catheter group was93.02%(40 cases)and that in traditional group was 7.7%(3 cases),and there was a significant difference between the two groups(P<0.05).43 patients in the transanal drainage catheter group were successfully placed.After successful catheter placement,43 patients continued decompression,cleaned the intestine and completely relieved obstruction.They were able to eat normally.One week later,they were transferred to surgery for one-stage resection of tumors.In the traditional treatment group,37 cases underwent emergency staging operation,and only two cases underwent one-stage resection and anastomosis.The total complications were 2.33%in the transanal catheter group and 20.51%in the traditional treatment group.There was significant difference between the two groups.There were 2 cases(5.12%)of incision infection in the traditional treatment group and 1 case(2.33%)in the catheter group.There were 1 case of abdominal abscess,periostal inflammation,cardiovascular accident and respiratory failure in the traditional treatment group,but none in the catheter group.The short-term mortality rate of the traditional treatment group was 5.12%,and there were no death cases in the catheter group.The total hospitalization time was shorter and the total hospitalization cost was lower in the treatment group of transanal drainage catheter.There was significant difference between the two groups.Conclusion:Transanal drainage catheter is a conservative medical treatment,which can prepare for surgery and relieve symptoms for patients with colorectal cancer who have no chance of operation.It is a safe and effective method.Compared with the traditional enema,the transanal drainage catheter is more advantageous in preparing patients for surgery and reducing the risk of complications.The application of transanal drainage catheter is aimed at low intestinal obstruction caused by colorectal cancer,and the effect of colon obstruction caused by left colon cancer is better,which is worthy of clinical application and promotion.
