Objective:Drug abortion is an effective,safe and simple non-surgical therapy,but the main side effects of drug abortion are incomplete drug flow,long vaginal bleeding and even large amount of bleeding after abortion.The main causes of this problem are poor uterine contraction and endometrial repair disorders.Traditional Chinese medicine?TCM?has unique advantages in promoting the elimination of intrauterine residues and the repair of endometrium after medical abortion to achieve hemostasis.The oral liquid of new shenghua decoction decoction?NSHD?is based on the traditional Chinese medicine biochemical decoction,and made by adding or subtracting formula.It is mainly used for the treatment of pregnant women after normal postpartum or medical abortion.Comparison with other drugs in curative effect is lacking.To compare and analyze the curative effect of NSHD,leonurus and oxytocin by observing the daysand amount of vaginal bleedingand the TCM syndromes,so as to provide a high-quality solution for preventing and treating the hemorrhage after drug abortion.Methods:The patients with drug abortion in normal early pregnancy in our hospital,aged20-35 years old,with menopause for 36-47 days,were confirmed to be pregnant by urine test,b-ultrasound and gynecological examination,and agreed to drug abortion without contraindications.The combination of mifepristone with misoprostol was used for drug abortion.After the pregnancy sac was discharged,the patients were randomly divided into four groups:Blank group,oxytocin group,leonurus group and NSHD group,which were given different treatments.Cases in blank group?46?were not treated after embryo sac ejection.In the oxytocin group?46 cases?,oxytocin was intramuscularly injected once?10U?after embryo sac expulsion.In leonurus group?52 cases?,Leonurus granules was orally given,1 bag adding 2 times of hot watereach time in the morning and evening every day for a course of 7 days.In the group of NSHD?56 cases?,warming NSHD was orally given 25ml of each time,which was taken twice in the morning and evening every day for a course of 7 days.Patients were re-examined on the 7th and 14th days after abortion respectively,and the changes of progesterone and HCG in blood were detected.B ultrasonography was used to detect the presence of residues in uterine cavity and uterine size.The number of days and the amountof vaginal bleeding?compared with the amount of menstruation?,the time from embryo sac to menstruation were recorded.In addation,with reference to the guiding principles for clinical research of new Chinese medicine in the treatment of lochia after delivery,the efficacy criteria of TCM symptoms were formulated and TCM syndrome score was conducted to further determine the therapy efficacy.The above indicators were compared among each group.Results:The duration of vaginal bleeding after miscarriage was obvious difference in the four groups:Only 7.02±1.68 days in NSHD group,8.75±1.51 days in leonurus group,10.97±1.46 days in oxytocin group,12.46±1.54 days in blank group.It is significantless in NSHD and herba leonuri groups than that in oxytocin and blank groups?P<0.01?,and there is significant difference between the NSHD and herba leonuri group?P<0.01?.Thedays of vaginal bleeding less than 7 dayswas as apparent effective and 8-14 days was as effective.In blank group,the apparent effective rate was 17.39%and effective rate was 82.60%,while in oxytocin group was 36.96%and 63.04%,in leonurus group accounted was 50%each,in NSHDgroup was 66.07%and 33.93%respectively.The amount of vaginal bleeding in each treatment group was less than that in blank group?P<0.01?,while the amount of vaginal bleeding in the NSHD group was the least,which was significantly different from that in the oxytocin group and the leonurus group?P<0.01?.Comparison of curative effect results of each group showed that:leonurus group and NSHD group were both better than oxytocin group?P<0.05 and P<0.01?.The content of progesterone and HCG in blood of pregnant women in each group were detected before abortion,on the 7th and 14th days after abortion respectively.There was no significant difference in progesterone or HCG content in the four groups before drug abortion?P>0.05?.After abortion,the progesterone or HCG content in each group decreased significantly on the7th and 14th days in turn.By their own before and after comparison,there were significant differences inall of the groups?P<0.01?.On the 7thh days,the content of progesterone was significant differences in each treatment group compared with that in the blank group?P<0.01?,there was also a statistical difference between theoxytocin group and the leonurus group orthe NSHD group?P<0.05?,there was no significant difference between the leonurus group and the NSHD group?P>0.05?.On the14thdays,the content of progesteronewas no statistical significance in all of the groups?P>0.05?.The comparison of HCG on the 7thh days after abortion:Compared with the blank group,there was no significant differences in theoxytocin group?P>0.05?,while in the leonurusorthe NSHD group it had statistical differenc?P<0.01?.Compared with the oxytocin group,there was no significant difference inthe leonurus group?P>0.05?,but inthe NSHD group it had statistical differenc?P<0.01?,and there was also a statistical differenc between the NSHD group and the leonurus group?P<0.01?.Comparison of the three uterine diameteres:There were no significant differences in the uterine dimetersof length,width and thickness and their sum in the four groups before drug abortion?P>0.05?.After abortion,the three dimeters in each group decreased in a different degree on the 7th day.By their own before and after comparison,there were significant differences inall of the groups?P<0.01?.The uterine dimetersof length,width and thickness in the treatment groups had Some degree of difference,but there was no statistical difference?P>0.05?.The sum of the three dimeters wassmallerin each treatment group than that in the blank group,but only in NSHD group it had statistical difference with the blank group?P<0.01?,there had no significant difference between other groups?P>0.05?.There was no significant difference in the time of menstruation recurrence between the groups?P>0.05?.Conclusion:The oral liquid of new shenghua decoction can significantly reduce the amount and time of vaginalbleeding,relieve TCM syndrome,promotethe uterine recovery after abortion,and the effectsare better than leonurus and oxytocin.
