Objective:To investigate the relationship between serum fetuin A levels and different stages of diabetic nephropathy(DN)in patients with type 2 diabetes mellitus(T2DM).Methods:188 patients with type 2 diabetes who were treated in our hospital from August 2017 to August 2018 were selected.T2 DM patients were divided into three groups according to urinary albumin/creatinine ratio(ACR): normal albumin with ACR<3.39mg/mmol Urinary(NA)group;3.39? ACR < 33.9 mg / mmol microalbuminuria(MA)group:ACR ? 33.9 mg / mmol of large albumin urine(OA)group.In addition,124 healthy subjects in our hospital were selected as the normal control(NC)group,and data such as height,weight,waist circumference,and hip circumference were measured,and body mass index(BMI)and waist-to-hip ratio were calculated: urine albumin and urine creatinine were measured.Calculate ACR,measure fasting blood glucose(FBG),postprandial 2h blood glucose(2hPG),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low protein cholesterol(LDL-C)Glycated hemoglobin(HbA1c),fetuin-A,adiponectin(ADP)and interleukin-1 ?(IL-1 ?)and statistical analysis of these indicators.Results:According to the order of NC group?NA group?MA group?OA group,serum fetuin A and IL-1? levels and ACR increased in turn,and ADP levels decreased in turn.The overall comparison between groups and the difference between the two groups were statistically significant.Significance(P<0.05).The level of fetuin A in T2 DM patients was positively correlated with ACR,fasting blood glucose,postprandial 2 h blood glucose,interleukin-1?levels and disease course(P<0.01),and negatively correlated with adiponectin level(P<0.01).Analysis showed that fetuin A,interleukin-1?,adiponectin and disease course were independent influencing factors of ACR(P <0.01).Conclusion:with the increase of ACR,serum fetuin A level gradually increased,The more severe the diabetic nephropathy,the higher the serum fetuin A level.Fetuin A is negatively correlated with adiponectin,suggesting that adiponectin may exist as a cytokine that protects the kidney.Fetuin A is positively correlated with IL-1?,and IL-1? acts as a major regulator of inflammation in various tissues,suggesting that fetuin A is involved in the inflammatory response of DN development.Multiple regression analysis showed that fetuin A levels,ADP levels,IL-1 ? levels,and disease duration were the influencing factors of ACR,ie,the influencing factors of DN.
