Cross-Cultural Adaptation And Applicative Research Of Inventario Del NIvel De Cuidados Mediante IndicAdores De Clasificación De Resultados De Enfermería(INICIARE)

Objective:(1)The study introduces the Inventario del NIvel de Cuidados mediante Indic Adores de clasificación de Resultados de Enfermería based on the nursing outcomes classification and Henderson's basic human needs theory to China with a rigorous cross-cultural adaptation,and a Chinese version of Inventario del NIvel de Cuidados mediante Indic Adores de clasificación de Resultados de Enfermería suitable for Chinese cultural background is formed finally.(2)To provide a reference for improving the quality of care and promoting the rational allocation of nurses resources,the Chinese version of the assessment is used to investigate the stroke inpatients in two third-grade general hospitals in Taiyuan City,Shanxi Province,to understand the current status of nursing care dependency of stroke patients and to analyze the influencing factors respectively.Methods:(1)The cross-cultural adaptation of the Inventario del NIvel de Cuidados mediante Indic Adores de clasificación de Resultados de Enfermería Firstly,we contacted with Dr.Ana Maria Porcel-Galvez through e-mail for the authorization.The currently accepted steps of the adaptation of the scale "forward translation-integration-back translation-experts review/experts consultation-preinvestigation's Brislin Translation Model were taken for cross-cultural adaptation of INICIARE,and the Chinese version of the INICIARE was tested psychologically in340 stroke patients admitted to the third-grade general hospital of Taiyuan City.The scale item was analyzed with the critical ratio method and the correlation coefficient method.And the validity analysis was made using the content validity and the calibration validity respectively.The reliability was divided into split-half reliability,internal consistency and test-retest reliability after one week,and a Chinese version of INICIARE was formed finally.(2)The applied research of the Inventario del NIvel de Cuidados mediante Indic Adores de clasificación de Resultados de Enfermería in patients with stroke A sample of 386 stroke hospitalized patients in two third-grade general hospitals in Taiyuan City,Shanxi Province were selected as a research object.The questionnaire comprises the general questionnaire of patients and the Chinese version of INICIARE.The collected data was analyzed through the descriptive analysis,univariate analysis and multiple stepwise regression and so on.The nursing care dependency and influencing factors of hospitalized patients with stroke were analyzed.Results:(1)The results of cross-cultural adaptation In the process of cross-cultural adaptation,the first round of Delphi experts consulted their positive coefficient,authority coefficient and coordination coefficient were 94.44%,0.908 and 0.360;the second round of Delphi experts consultation results were 100%,0.926 and 0.373;p < 0.001.Nurses took about 15 minutes to evaluate the patient.Reliability and validity test: The Cronbach's ? coefficient of the Chinese version of INICIARE is 0.985,and the Cronbach's ? coefficient of each dimension is0.838~0.978.And the split-half reliability is 0.918,the split-half reliability of each dimension is 0.836~0.976.The test-retest reliability is 0.973 after one week,and the test-retest reliability of each dimension is 0.778~0.949.The average S-CVI of the Chinese version of INICIARE is 0.99,and the I-CVI of each item changes from 0.88 to 1.00.The correlation validity of the calibration is r=0.925(p<0.01).The correlation coefficient between the scores of each dimension and the total score of the scale is 0.653~0.906(P<0.01).The final Chinese version of INICIARE includes 14 aspects and 57 secondary indicators.(2)The results of applicative research In the applied research,the results show that the mean ± standard deviation of the total score of patients' nursing care dependency is 173.28±47.808,the lowest score in each dimension is 3.51±1.030,and the highest score is 33.92±9.694.In the analysis of influencing factors,admission way,disease type,and nursing grading are the main factors affecting the total score of care dependency of stroke patients and the different factors in each dimension.In addition,education level and age are the main influencing factors affecting the total score of nursing dependence.Conclusion:(1)The Chinese version of INICIARE index covers great comprehensiveness,good validity and reliability,and the content is simple and easy to understand.It is suitable for assessing the level of nursing care dependency of stroke patients in China and promoting it in clinical use.(2)The mean value of the nursing care dependency score of the inpatients with stroke in the third-grade general hospital of Taiyuan City is 173.28±47.808,among which the reasonable diet has the highest score of nursing care dependency and the lowest the nursing care dependency of body temperature.At the same time,among the influencing factors affecting nursing care dependency of patients,admission ways,disease types,nursing grades,education level and age are the main influencing factors affecting patients' nursing care dependency.Therefore,the nursing managers should make rational allocation of positions,adjust the staff's configuration structure according to the status of the patient's nursing care dependency and its influencing factors,give full play to the advantages of clinical nurses,realize the high-quality development of nursing resources,and effectively provide clinical nursing services that provide patients with peace of mind,efficient nurses,and satisfactory government.
