BackgroundCurrently,the specialized hospitals of infectious diseases are facing the development bottleneck.Under this circumstance,the "comprehensive" development mode of the specialized hospitals of infectious diseases is an effective way to get rid of the dilemma.It is a new development mode for the specialized hospital of infectious diseases and the Triple A General Hospital to form the hospital group and implement the new model of inter-hospital trusteeship under the leadership of the administrative committee.ObjectiveIt is to conduct the research and analysis on the management mode,operation mechanism,and operation effects of the specialized hospital of infectious diseases in hospital group,describe the distribution of evaluation indexes before and after the reform,evaluate the effect of reform comprehensively,find out problems and deficiencies,and sum up experiences and practices so as to promote the development of the "comprehensive"mode of the specialized hospital for infectious diseases in a stable,coordinated,and sustainable way.Subjects and methodsIt is to apply a specialized hospital's statistics system of the personnel,science&education,and medical record as well as the questionnaire survey of the staff in the reform of collectivization to collect the data of manpower,medical service,scientific research,medical expenses,and staff cognition before and after the reform of the collectivization of the hospital.During this course,Exce12007 software is applied to collate data,and SPSS 19.0 software is applied for statistical analysis;the chi-square test is also applied;TOPSIS(approximation to ideal value),RSR(Rank Sum Ratio),and the entropy weight method are applied for statistical analysis.Results1.From 2016 to 2018,the numbers of the staff were 552,607,and 637,respectively.The number of the staff increased continuously.There were significant differences in the composition of highly educated personnel in different years(P<0.05).There were not significant differences in the distribution of positional titles and ages(P>0.05).2.From 2016 to 2018,the rate of operation increased with time continuously(trend P<0.001).The rate of clinical pathway entry was 7.86%from January 2016 to June 2016.However,it increased to 51.39%from January 2018 to June 2018,which was an increase of 43.53 percentage points.The rate of clinical pathway entry increased with time(trend P<0.001).There were no linear relationships among rescue success rate,cure rate,and the year(all P>0.05).Through the fuzzy joint evaluation method of TOPSIS and RSR,it was concluded that the level of medical service in a special hospital was improving continuously from 2016 to 2018.Among it,the comprehensive evaluation result of medical service from January 2018 to June 2018 was the best,and the comprehensive evaluation result of medical service from January 2016 to June 2016 was the worst.3.From 2016 to 2017,the number of the published theses increased by 52.However,the high-quality theses accounted for less.The differences were not statistically significant in the composition of SCI,core journals,and general journals published in clinical,medical and administrative departments in different years(all P>0.05).Moreover,the differences in the composition of SCI,core journals,and general journals of all junior,middle,and senior staff was not statistically significant in different years(all P>0.05).4.The entropy weight method was applied to evaluate medical expenses.The results showed that the results(from the best to the worst)were as follows respectively:from July 2017 to December 2017,January 2018 to June 2018,January 2017 to June 2017,July 2016 to December 2016,and January 2016 to June 2016.5.The results of the questionnaire survey showed that the difference in the cognition of whether the group reform could bring benefits was statistically significant(P<0.05).There were 44.51%of the staff who thought that the group reform could bring benefits to the development;24.14%of the employees thought it couldn't,and 31.35%of the staff held a wait-and-see attitude.The result of the staff on whether the collectivization reform would help the transformation and development of hospitals showed that 52.04%of the staff thought that the collectivization reform could help the transformation and development of hospitals;18.81%of the staff thought it could,and 29.15%of the staff held a wait-and-see attitude.The difference in attitude among different occupational types was not statistically significant(P>0.05).Conclusions1.During the three years,the number of employees has been increasing.Among it,the number of highly educated people has increased obviously,and the professional title and age composition have not changed obviously.2.The effect of comprehensive medical service was getting better and better.Among it,the comprehensive evaluation result of medical service from January 2018 to June 2018 was the best,and the comprehensive evaluation result of medical service from January 2016 to June 2016 was the worst.3.The number of the published papers increased year by year,but the findings of high-quality scientific research were not enough;the economic benefits were improved obviously,and the burden of patients' disease was increased.4.The staff's recognition for the hospital collectivization reform was not high.
