Aims: Investigate that three detection methods of nasopharyngeal mucosa cell stability FH detection,fiber nasopharyngoscopy and EB virus serological examination in diagnosis of nasopharyngeal carcinoma independently and jointly.Methods: A total of 108 outpatients and inpatients from the Department of Otorhinolaryngology,Changsha First Hospital from June 2017 to June 2018 were selected.All subjects underwent nasopharynx biopsy.The pathological examination was the gold standard and the diagnosis will be confirmed.For the patients with nasopharyngeal carcinoma and precancerous lesions,54 patients were divided into observation group.Patients with normal and benign lesions of the nasopharynx were divided into control group,FH detection of nasopharyngeal mucosal cell stability,fiber nasopharyngoscopy and EB virus serology.Three kinds of examination methods were used to examine the observation group and the control group,and the positive and negative results of the three detection methods were observed.The results of the three methods were recorded and statistical analysis was performed by SPSS22.0 software,and three tests were calculated.Sensitivity,specificity,false positive rate,false negative rate,positive likelihood ratio,negative likelihood ratio,diagnostic index,positive predictive value,negative predictive value,conformity,crude coincidence rate,etc.The value of detection method in nasopharyngeal cancer screening,and the sensitivity,specificity,false positive rate,false negative rate positive likelihood ratio,negative of four combined detection schemes by calculating three detection methods Evaluation criteria such as likelihood ratio,diagnostic index,positive predictive value,negative predictive value,conformity,and crude coincidence rate were evaluated to evaluate the value of combined detection in nasopharyngeal cancer screening.Results:(1)Among the 54 patients in the observation group diagnosed with nasopharyngeal carcinoma and precancerous lesions,the positive rates of FH screening,EBV serum VCA-IgA detection,and fiber nasopharyngoscopy were 85.2% and 79.6%,respectively.90.7%.Among the 54 patients in the control group diagnosed as normal or benign lesions of the nasopharynx,the positive rates of FH screening,EBV serum VCA-IgA and fiberoptic nasopharyngoscopy were 24.0%,27.8%,18.5%,respectively.In the observation group,X2=2.641,P=0.267>0.005.There was no statistical difference in the detection rate of the three detection methods in the observation group.In the control group,X2=1.306,P=0.520>0.005,three detections There was no statistical difference in the detection rate difference between the two groups in the control group.(2)FH screening,EB virus serum VCA-IgA detection,fiberoptic nasopharyngoscopy clinical screening evaluation commonly used indicators: sensitivity were 85.2%,79.6%,90.7%;specificity: 76.0%,72.2%,81.5%;false positive rates were: 24.0%,27.8%/18.5%;false negative rates were 14.8%,20.4%,and 9.3%,respectively;positive likelihood ratios were 3.54,2.86,4.90,respectively;negative likelihood ratios were respectively The diagnostic indices were 161.2%,151.8%,and 172.2%,respectively;the positive predictive values were 78.0%,74.1%,and 83.1%,respectively;the negative predictive values were 82.7%,78.0%,and 89.8%,respectively.(3)When FH screening,Epstein-Barr virus serum VCA-IgA detection and fiber nasopharyngoscopy were combined,the sensitivity and false positive rate were lower than those of the single detection,but the specificity and false negative rate were compared with those of the individual detection.Raised.The combination of any two methods was the highest sensitivity(77.8%)with the FH detection of nasopharyngeal mucosal cell stability combined with EB virus serology;the highest specificity of the nasopharyngeal mucosal cell stability FH combined with electronic nasopharyngoscopy(90.7%).The combined detection sensitivity of the three was significantly reduced(66.7%),but its specificity was increased(92.6%).Conclusion: Nasal pharyngeal mucosal cell stability FH detection combined with Epstein-Barr virus VCA-IgA detection method has high sensitivity and specificity for the diagnosis of nasopharyngeal carcinoma.Because the FH screening test reagent product is simple,economical,rapid,and efficient,it is more suitable for extensive screening.Combined with Epstein-Barr virus Epstein-Barr virus VCA-IgA detection can improve the detection rate of nasopharyngeal carcinoma in the population.This method is simple and feasible.Suitable for clinical promotion applications.
