Objective: An animal model of elevated intra-abdominal pressure was established to simulate the increase of intra-abdominal pressure in obese patients.The effect of elevated intra-abdominal pressure on renin-angiotensin system and glucose metabolism was studied.The chronic intra-abdominal pressure elevation model was established with intermittent injection of minerals oil in rat abdominal cavity.Observation of the effects with elevated intra-abdominal pressure on hypertension related indicators renin,angiotensin I(Ang I),angiotensin II(Ang II)and glucose metabolism related indicators fasting blood glucose(FBG),glucose tolerance,insulin(INS).Methods: In experiment 1,18 SD rats were randomly divided into normal saline group(n=9)and mineral oil group(n=9).The normal saline group was injected with normal saline for 3 consecutive days,30 ml,40ml,30 ml,respectively.The mineral oil group was injected with the same amount of mineral oil in the same way.Intra-abdominal pressure wasmeasured and cardiac blood were taken on the 5th day to measure liver and kidney function and electrolytes in the two groups,and pathological changes of liver?kidney were observed.In experiment 2,20 rats were randomly divided into normal saline group(n=10)and mineral oil group(n=10).Rats in mineral oil group were injected intermittently with mineral oil to establish a rat model of increased intra-abdominal pressure.Rats were injected every 3 days,each time 15 ml,10 times,a total of 150ml;Rats in the saline group were injected intermittently with the same amount of normal saline,every 3 days,each time 15 ml,10 times,a total of 150 ml.Five weeks after the start of the injection,intra-abdominal pressure was measured,and serum renin,AngI,AngII,insulin concentration,fasting blood glucose level and glucose tolerance function were measured,and pathological changes of liver?kidney and pancreas were observed.Results:1.In the short-term intra-abdominal pressure elevation model,the intra-abdominal pressure of the mineral oil group was significantly higher than that of the saline group(p<0.001);2.In the short-term intra-abdominal pressure elevation model,compared with the saline group,the alanine aminotransferase in the mineral oil group decreased,and other indicators of liver and kidney function and electrolyte,liver and kidney histopathology in the mineral oil group did not change significantly;3.In the long-term intra-abdominal pressure elevation model,compared with the saline group,the levels of renin(p<0.05)and AngI(p<0.01)in the mineral oil group were significantly lower,and Ang II was not significantly different in the two groups.difference;4.In the long-term intra-abdominal elevation model,compared with the saline group,the mineral oil group insulin(p<0.05)and fasting blood glucose level(p<0.05)were significantly reduced,and half an hour after glucose challenge(p<0.005).And 1 hour(p<0.05)blood glucose levels were significantly increased,2 hours blood glucose levels were not significantly different between the two groups;5.In the long-term intra-abdominal elevation model,compared with the saline group,HE staining of the liver tissue,kidney tissue and pancreatic tissue of the mineral oil group showed a significant increase in congestion(p<0.05).The percentage of pathological changes in renal interstitial edema and tubular vacuolar degeneration in renal tissue increased.Conclusion:1.This experimental study successfully constructed a rat model of elevated chronic intra-abdominal pressure,which can be used to simulate changes of abdominal pressure during obesity,pregnancy,and ascites.2.The increase of intra-abdominal pressure in rats reduced the concentration of renin and Ang I,insulin and fasting blood glucose,and increased the blood glucose level after half-hour and 1 hour after glucose stimulation,which may be related to the obvious blood injury of liver,kidney and pancreas caused by the increase of intra-abdominal pressure in rats.
