Objective: By understanding the correlation between the pathological characteristics of early gastric cancer and the degree of tissue differentiation,we can provide theoretical basis for formulating more accurate treatment of early gastric cancer.Methods: This study retrospectively analyzed 93 patients with early gastric cancer diagnosed by pathological examination after radical gastrectomy or endoscopic treatment in the Second People's Hospital of Yueyang City from January 2004 to December 2018.The clinicopathological characteristics of early gastric cancer and the influencing factors of different differentiation degrees were analyzed.Age,sex,Helicobacter pylori infection,smoking,tumor size and swelling were related.Twelve indicators,including tumor location,endoscopic gross classification,histological classification,depth of invasion,lymph node metastasis,vascular invasion,ulcer or not,were analyzed by single and multiple factors.Results:1.The tumors of early gastric cancer are mainly located in the lower part of the stomach,with flat/depressed endoscopic manifestations.The pathological types are mainly poorly differentiated adenocarcinomas,and the undifferentiated ones are more than the differentiated ones.2.Signet ring cell carcinoma is more common in women(P<0.001)and patients younger than 60 years old(P=0.005);signet ring cell carcinoma is also more common in women than poorly differentiated adenocarcinoma(P=0.012),the location of tumors is more common in the middle of the stomach(P=0.005),the depth of invasion is often limited to the mucosa(P=0.003),and endoscopic gross typing is more common in flat/depressed type(P=0.019).3.CA 19-9 and CEA are less valuable in early gastric cancer.4.Univariate analysis showed that age,sex,tumor location,tumor size,endoscopic gross typing and depth of tumor invasion were related to the degree of tissue differentiation.5.Multivariate analysis showed that age,sex,tumor size and endoscopic gross typing were independent risk factors for tissue differentiation of early gastric cancer.Conclusion:1.Early gastric cancer mainly occurs in the lower part of the stomach,with flat/depressed endoscopic features;low differentiated adenocarcinoma is the main pathological type,undifferentiated type is more than differentiated type;CA 19-9 and CEA are less valuable in early gastric cancer.2.The degree of tissue differentiation of early gastric cancer is related to age,sex,tumor size and endoscopic gross typing.Young female patients with undifferentiated type,especially early signet ring cell carcinoma,should be careful when choosing minimally invasive treatment.
