Identification And Analysis Of Traditional Chinese Medicine Constitution In Infertility Patients With Abnormal Sperm DNA Fragments

ObjectiveGuided by the theory of traditional Chinese medicine constitution,this paper aims to explore the characteristics of constitution of Traditional Chinese Medicine identification and distribution in infertility patients with abnormal sperm DNA fragments,and analyses the relationship between the main parameters of sperm and the constitution of traditional Chinese medicine,so as to provide some reference data for the treatment and maintenance of infertility patients with abnormal sperm DNA fragments.MethodsThe 118 infertile patients from July 2018 to December 2018 in the department of reproduction or andrology of Guangdong Provincial Hospital of Traditional Chinese Medicine were studied.The average age was 34.24±6.020 and the average infertility time was 2.875±1.0397.Recovering the TCM constitution identification and analysis scale,recording the general information of patients,the detection value of sperm DNA fragments and the semen Analysis.Analyzing the distribution characteristics of constitution types in patients with abnormal sperm DNA fragments,the relationship between main constitution types and patients' general data,sperm DNA fragment index,semen related parameters by relevant statistical processing.Results1?Overall distribution of constitution types in traditional Chinese medicine:Among the 118 respondents of infertile patients with abnormal sperm DNA fragments,75 cases(63.56%)showed single TCM constitution type,33 cases(27.97%)showed mixed TCM constitutiont type and 10 cases(8.47%)showed gentel TCM constitutiont type;The frequencies of single TCM constitution type were damp-heat constitutiont in 19 cases(16.10%),qi deficiency constitutiont in 16 cases(13.56%),phlegm-dampness constitutiont in 12 cases(10.17%),qi depression constitutiont in 11 cases(9.32%),Yin deficiency constitutiont in 10 cases(8.47%),blood stasis constitutiont in 4 cases(3.39%),special constitutiont in 2 cases(1.70%)and yang deficiency in 1 case(0.85%).2?The distribution of mixed TCM constitutiont type:Among the 33 mixed constitutions,6 cases(18.18%)were phlegm dampness and qi deficiency,followed by phlegm dampness and qi depression in 5 cases(15.15%)>dampness heat and yin deficiency in 4 cases(12.12%)>qi deficiency and qi depression,Yin deficiency and qi deficiency,blood stasis and yang deficiency in 3 cases each(9.09%)>Yang deficiency and phlegm dampness,Yang deficiency and qi deficiency,special constitutiont and dampness-heat,qi deficiency and yang deficiency in 2 cases each(6.06%)>damp heat and blood stasis in 1 case(3.03%).3?Among the 118 infertile patients with abnormal sperm DNA fragments,There were 15 cases(13%)with normal semen,103 cases(87%)with abnormal semen containing 10 cases(8%)with asthenospermia,71 cases(60%)with weak malformed sperm,18 cases(15%)with oligo-astheno-teratospermia,3 cases(3%)with oligoasthenosperm,1 case(1%)with oligomalformed sperm.4?There was no significant difference between the main TCM constitution types and weak malformed sperm,oligo-astheno-teratospermia(P>0.05).5?The correlation analysis showed that the main constitution types(damp heat,qi deficiency,phlegm dampness,qi depression,mixed constitutiont)were correlated with sperm DNA fragmentation rate(R=0.478,P<0.001),sperm total motility(R=0.648,P<0.001),the rate of progressively motile sperm(R=0.773,P<0.001),Sperm concentration(R=0.482,P<0.001),but no significant correlation with the rate of morphologicaliy nomal sperm(P>0.05).6?Regression analysis showed that the effect of qi deficiency constitution on sperm DNA fragments was the most significant,which was higher than other constitution types(?=15.313,P<0.001).qi depression constitutiont had the most significant effect on the total motility of sperm and the proportion of forward motile sperm,which was higher than other types(?=11.701,?=31.948,P<0.001).The effect of damp-heat constitution on sperm concentration was significant,higher than that of other constitution types(?=-126.892,P<0.001).Conclusion1?The most common constitutional types of infertility patients with abnormal sperm DNA fragments are constitution of damp heat,qi deficiency,phlegm dampness,qi depression and mixed type.2?Damp heat quality,qi deficiency quality,phlegm dampness quality,qi depression quality,mixed constitution and sperm DNA fragmentation rate,sperm total motility,the rate of progressively motile sperm,sperm concentration were correlated,but not with the rate of morphologicaliy nomal sperm.3?Qi deficiency constitution has the most significant effect on sperm DNA fragments;qi depression constitution has the most significant effect on total sperm motility and forward motility sperm proportion;and damp-heat constitution has the most significant effect on sperm concentration.
