ObjectiveMyasthenia gravis(MG)is a rare autoimmune disease that is difficult to treat.At present,it is believed that antibodies against acetylcholine receptors act on the postsynaptic membrane of skeletal muscle neurons,resulting in conduction dysfunction.The disease can be caused by a variety of reasons such as infection,fatigue,female menstruation and so on,even the occurrence of crisis,can endanger the life of patients at any time.Professor Tietao Deng,a master of traditional Chinese medicine,is one of the leading doctors of traditional Chinese medicine.Based on his years of clinical experience,he made up his own prescription of Qiang Ji Jian Li Yin.Studies have shown[1]that it can be used to treat myasthenia gravis by slowing down the breakdown of acetylcholine receptors.It has also been found[2]that this prescription can achieve the effect of treating myasthenia gravis by regulating cellular immunity in the body.And thymus is the largest immune organ in human body,there are a large number of immune cells,our study is to further systematic review of patients with myasthenia gravis thymus abnormalities,through immunohistochemical method to observe the thymusimmune cells called T cells subgroup and its secretion factor IL-6,ROR ?T,Foxp3,IL-17,the expression of TGF-?,observation of patients with long-term use of traditional Chinese medicine(TCM)and patients with not long-term use of traditional Chinese medicine,the differences of cytokines in the body,and cytokine expression differences between different TCM syndrome types,To investigate the effect of Bu Zhong Yi Qi Tang decoction on TCM and the immunological mechanism of TCM in patients with myasthenia gravis after thymectomy.Method1.Collect the patients diagnosed as MG in the first affiliated hospital of guangzhou university of traditional Chinese medicine from September 2011 to June 2017,who underwent thymectomy in our hospital due to thymic abnormalities,and were diagnosed as thymoma or thymic hyperplasia in postoperative pathological sections.The pathological sections of the patients were borrowed for the examination of the immunohistochemical protein IL-6,ROR?T,Foxp3,IL-17,and TGF-p.2.By recording patients' gender,age,classification,TCM syndrome type,TCM dosage,course of disease,combination of basic diseases,preoperative crisis,postoperative and other relevant data,the common TCM characteristics of MG patients with thymus abnormalities were analyzed,and the effects of featured TCM treatment were compared.3.The experimental results of immunohistochemical data were compared with each other,and statistical analysis was carried out between the experimental results and the observation indexes including TCM syndrome types and the situation of taking TCM,to explore the differences in the expression of various cytokines and the relationship between different TCM syndrome types and cytokines.ResultsPatients with thymic hyperplasia and thymoma showed significant differences in age,age of onset,and course of disease(P<0.05),and the expression of IL-6 in the two groups was significantly different(P<0.05).The expression level in thymic adenoma was higher than that in thymic hyperplasia.There was a difference in TCM syndromes of patients with thymus abnormalities between the two groups(P<0.05),and there was no significant difference in the expression of cytokines of different TCM syndromes in MG patients(P>0.05).The expression of TGF-? was significantly different in different MG genotypes(P<0.05),with the lowest expressionin ocular muscle types.There was a difference between preoperative disease course and long-term use of traditional Chinese medicine(P<0.05),and there was no significant difference between different types of MG and traditional Chinese medicine.There was a positive correlation between MG typing and postoperative crisis.There was a positive correlation between RORyT and FOXP3.Postoperative infection,crisis and hospitalization time were not related to the expression of cytokines.ConclusionsEmpirical studies have found that IL-6 is highly expressed in thymus of MG patients,which has a certain impact on the condition of MG,aggravating the development of MG disease.TGF-? is differentially expressed in different MG genotypes,with low expression in ocular muscle genotypes and increasing expression in other genotypes,indicating that the condition of MG can be controlled by blocking the expression of TGF-? and IL-6.For MG patients with a longer course of disease,they are more inclined to take traditional Chinese medicine,which reflects the definite curative effect of traditional Chinese medicine on MG patients.Two groups of patients with thymic abnormalities with MG in the differences between the TCM syndrome types,the late for the study of TCM,syndrome differentiation,to explore Chinese medicine effectivecomponent,giving full play to the advantages of traditional Chinese medicine,combination of pharmacology at the same time,explore block oWreduce the expression of cytokines related new drugs,targeted therapy provides simple idea of the MG.
