| ObjectiveTo observe the clinical efficacy and safety of Liang Xue Jie Du Tang on treating the bite by Trimeresurus gramineus,and its effects on coagulation indexes,include platelet(PLT),activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT),fibrinogen(Fg)and so on.MethodsEighty eligible patients with snake bite by Trimeresurus gramineus were randomly divided into treatment group(40 cases)and control group(40 cases).The treatment group was treated with routine western medicine plus Liang Xue Jie Du Tang,while the control group was treated with routine western medicine.The clinical symptoms,gender,age,severity of illness,and PLT,PT,APTT,TT,Fg values of the two groups before treatment were recorded.The relaxation of swelling and pain,hospitalization days,hospitalization expenses,overall curative efficacy and PLT,PT,APTT,TT,Fg values of the 7th day were recorded after treatment.SPSS20.0 software was used for statistical analysis of observation indicators.Independent sample t-test was used for comparison between measurement data groups,paired sample t-test was used for comparison within groups,chi-square test was used for comparison of counting data,and rank sum test was used for comparison of ordered variable data.Results1.There were no significant differences in age,sex,severity of disease and the values of PLT,PT,APTT,TT and Fg before treatment among 80 patients bitten by Trimeresurus granineus in this study.2.The clinical symptoms of 80 patients bitten by Trimeresurus gramineus before treatment were summarized and analyzed.The results showed that local swelling accounted for 100%,local pain accounted for 100%,local bleeding accounted for 68.75%,blue and purple skin or ecchymosis accounted for 43.75%,limb numbness accounted for 27.5%,and no obvious systemic symptoms were found.3.In this study,the relief time of swelling in treatment group(6.20±1.14d),control group(8.13±1.54d),the relief time of swelling in treatment group was shorter than that in control group(P<0.01);the relief time of pain in treatment group(5.15±1.44d),and the relief time of pain in control group(6.53±1.34d),the relief time of pain in treatment group was shorter than that in control group(P<0.01).4.The hospitalization days in the treatment group(8.05±1.11d)and the control group(9.43±1.47d)were significantly shorter than those in the control group(P<0.01);the hospitalization expenses in the treatment group(5291.45±846.36Y)and the hospitalization expenses in the control group(7737.67±1878.35Y),and the total costs in the treatment group were lower than those in the control group(P<0.01).5.In the treatment group,7 cases were cured,29 cases were markedly ef'fective,and in the control group,2 cases were cured and 27 cases were markedly effective.The marked efficiency in the treatment group was 90%and that in the control group was 72.5%.The curative effect in the treatment group was better than that in the control group(P<0.05).6.The average PLT of the treatment group and the control group decreased after treatment,but there was no statistical difference(P>0.05).After treatment,the prolongation of PT in the treatment group was shorter than that in the control group(P<0.01),the prolongation of APTT was shorter than that in the control group(P<0.05),the prolongation of TT was shorter than that in the control group(P<0.01),and the decline of Fg less than that in the control group(P<0.05).Conclusion1.The main clinical symptoms of snake bite by Trimeresurus grarmineus are local swelling,pain,bleeding,skin cyanosis or ecchymosis,limb numbness and other local symptoms.Swelling and pain are the necessary symptoms for clinical diagnosis,while dizziness,headache,panic,chest tightness and other systemic symptoms are rare.2.Liang Xue Jie Du Tang can significantly shorten the time of relieving swelling and pain of limbs,reduce the average hospitalization days,and reduce medical costs in the treatment of snake bite by Trimeresurus gramineus.3.Liang Xue Jie Du Tang has a good preventive and therapeutic effect on the prolongation of PT,APTT,TT and the decline of Fg caused by the bite of Trimeresurus gramineus,but it has no improvement on the decline of PLT.However,the specific mechanism of the effect of Liang Xue Jie Du Tang on coagulation function needs further experimental study.4.Liang Xue Jie Du Tang is an effective prescription for the treatment of snake bite by Trimeresurus gramineus,with a marked efficiency of 90%.There is no obvious toxic and side effects in the short-term course of treatment. |
