Objective:Alzheimer's disease(AD)is one of the most common types of senile degenerative diseases,and amnestic-type Mild Cognitive Impairment(aMCI)is considered to be the precursive performance of AD.AD and aMCI are called"senile dementia" in traditional Chinese medicine.This experiment is to study the hippocampal volume,MMSE and MoCA scores,and the distribution of TCM syndromes in pat ients with senile dementia.Compare the hippocampal volume and cognitive impairmentbetween different TCM syndrome types of the senile dementia patients,andexplore the shrinking degree of hippocampal volume and the level of cognitive impairment in shenyang deficiency syndrome.it provides a theoretical reference for emphasizing the important status of kidney-yang deficiency syndrome and the Fuyang Guben treatment ideas of AD and aMCI.Methods:A total of 121 patients with AD and aMCI(the senile dementia group)were enrolled.The syndrome differentiation of single syndrome was divided into different TCM syndrome types(53 cases of shenyang deficiency syndrome group,32 cases of blood stasis syndrome group,36 phlegm turbidity syndrome group).A total of 87 healthy elderly volunteers(the control group)who were recruited.Collect general information of all subjects,record the four-integration data of TCM,and evaluate the mental and psychological scales of all the integrators.All subjects underwent MRI examination of the brain.The original image images obtained by MRI were transferred to the workstation,and calculate the head,body,tail,and total volume of the hippocampus,and the resulting data results are normalized according to the formula.The head,body,tail,and total volume of the hippocampus were compared in all subjects.At the same time,the head,body,tail,total volume of the hippocampus and MMSE,MoCA scores were compared between the senile dementia group and the control group,and between thedifferent TCM syndrometyps groups.Andevaluating the correlation between the head,body,tail,and total volume of the hippocampus and MMSE and MOCA scores in he senile dementia group.Results:1.There was no significant difference in gender,age and education level between the control group and the dementia group,the different TCM syndrome types in the senile dementia group.(P>0.05).2.Compared with the volume of the left hippocampus,there was no significant difference in the head,body,tail and total volume of the right hippocampus of all subjects.(P>0.05).3.Compared with the control group,the head,body,tail and total volume of the bilateral hippocampus in the dementia group were significantly decreased(P<0.01).Compared with the control group,the MMSE and MOCA scores were significantly lower in the dementia group(P<0.01).4.The distributions of the TCM syndrome types in 121 Patients with senile dementiawere as follows:shenyang deficiency syndrome accounted for 43.80%(53/121),blood stasis syndrome accounted for 26.44%(36/121),and phlegm turbidity syndrome accounted for 29.75%(36/121).5.Among the different TCM syndromes of thesenile dementia patients,compared with the shenyang deficiency syndrome group,the head,body,tail and total volume of the hippocampus in the blood stasis syndrome group and the phlegm turbidity syndrome group were significantly increased(P?<0.01);Compared with the blood stasis syndrome group,there was no significant difference in the head,body,tail and total volume of the bilateral hippocampus in the phlegm turbidity syndrome group(P>0.05).6.Among the different TCM syndromes of thesenile dementia patients,compared with the shenyang deficiency syndrome group,the MMSE and MoCA scores of the blood stasis syndrome group and the phlegm turbidity syndrome group were significantly increased(P<0.01);compared with the blood stasis syndrome group,There was no significant difference in MMSE and MoCA scores of the phlegm turbidity syndrome group(P>0.05).7.Correlation analysis between bilateral hippocampal volume and MMSE and MoCA scores in patients with thesenile dementia:There was a significant correlation between the head,body,tail and total volume of bilateral hippocampus and MMSE and MoCA scores(P<0.01),and it was proportional.Conclusion:1.In the process of gradual transformation of the normal elderly to thesenile dementia,the parts and total volume of bilateral hippocampus were significantly reduced,and the degree of hippocampal volume reduction was synchronous,and the scores of MMES and MoCA were also significantly reduced.2.In the patientaswith senile dementia,the shenyang deficiency deficiency syndrome group had a greater reduction in the head,body,tail and total volume of the hippocampus than the blood stasis syndrome group and the phlegm turbidity syndrome group;the shenyang deficiency deficiency syndrome group wasworse in the level of cognitive function than the blood stasis syndrome group and the phlegm turbidity syndrome group;suggesting that we should pay attention to warming shenyang in clinical treatment of senile dementia.3.In the patientaswith senile dementia,there was a significant proportional correlation between the shrunken degree of the segmenta and total volume of bilateral hippocampus and the decline of cognitive function,suggesting that hippocampal structural changes may cause cognitive impairment.
