Effects Of Auricular Acupoint On Early Recovery Quality After Gynecological Laparoscopic Surgery

Objective:Based on the traditional Chinese medicine and the concept of accelerated rehabilitation in recent years,evaluating the effect of auricular acupoint on the quality of early recovery after gynecological laparoscopic surgery,this study observes the quality of early recovery after gynecological laparoscopic surgery,the degree of pain in patients at all the time points after surgery,the rate of postoperative nausea and vomiting,and the recovery of gastrointestinal function after surgery,whieh is helpful to understand t he feasibi lity and val idity of auricular acupoint pressing,to design a better research,and to provide data supports for clinical nursing.In order to improve the quality of early recovery of gynecological laparoscopic patients,we observe the recovery of patients after surgery,so we can explore effective and feasible methods to reduce postoperative discomfort and accelerate the recovery of patients.Methods:This study included 72 gynecological elective laparoscopie surgery patients admitted to Guangdong Traditional Chinese Medicine Hospital from March 2018 to October 2018.One patient was excluded because of the long operat ion time,and finally 71 patients were included.They were randomly divided into two groups:observation group(auricular point group)36 cases and control group(auricular point placebo-controlled group)35 cases.The observation group was given auricular acupoint intervention on the basis of routine medical treatment and nursing.The method was to inf'orm the patients of the purpose of this study by visiting the day before operation and to sign the informed consent.We checked the patient's aurieles,disinfected the patient ' s auricles with 75%ethanol,and selected Shenmen,Heart,Jiaogan,Stomach,Spleen,Subcortical,Uterus acupoints by car acupoint detector.We asked the patient to press the thumb and index finger pulpwith relative force,from light to heavy,until the patient has a sense of "getting qi" such as acid numbness and swelling.Ask patients to press each acupoint one day before operation,one minute each time,with even force and alternate pressing by both ears.Before the operation,the operator pressed each acupoint on the patient's auricles for one minute at a time,exerting force uniformly and pressing alternately with both ears.After returning to the ward,patients were asked to press five times a day(8:00,12:00,16:00,20:00,bedtime),one minute at each acupoint,alternately pressing both ears,and the ear acupoint stickers were removed on the third day after operation.The scores of 40 items of quality of recovery scale(QoR-40)were observed one day before operation and 48 hours after operation;the pain VAS scores of patients at different time after operation were observed;the incidence of nausea and vomiting after operation;indicators of recovery of gastrointestinal function after operation were recorded,including the first exhaust time,the first defecation time,the number of days in hospital and the satisfaction scores.Results:1.A total of 72 patients were included in this study.One of them was excluded because the operation time was too long and did not meet the inclusion criteria.Finally,71 patients were included,36 in the observation group and 35 in the control group.No adverse reactions and side effects occurred during the study.2.There was no significant difference between the two groups in general data:age,height,weight,operation time and anesthesia time(P>0.05).There was no significant difference in the total score of QoR-40 scale and each item score between the two groups before operation(P>0.05).There was no significant difference in heart rate and mean arterial pressure during operation(P>0.05).3.In terms of the total score of QoR-40 scale,48 hours after operation,the scores of the observation group was higher than that of the control group(P<0.05).Among them,the scores of emotional state,physical comfort and pain of' the observation group were higher than those of the control group 48 hours after operation,with stati stical significance(P<0.05).There was no significant difference in scores of psychological support and self-care ability between the two groups in 48 hours(P>0.05).4.Other indicators reflecting the quality of postoperative recovery:VAS scores of the observation group were lower than those of the control group at 4,12,24 and 48 hours of follow-up(P<0.05),and the VAS scores of the two groups were also significantly different with time(P<0.05);the incidence of nausea and vomiting in the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The difference was statistically significant(P<0.05);t he first exhaust time and first defecation time of' the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05).5.Hospitalization Days:There was no significant difference in hospitalization days between the two groups(P>0.05).6.Satisfaction Scores:The satisfaction scores of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:1.Auricular acupoints can improve the quality of recovery of patients undergoing gynecological laparoscopic surgery,which is reflected in the total scores,emotional state scores,body comfort score and pain scores of 40 post-operative recovery quality scale(QoR-40 scale)are higher than thos?of the control group.2.Auricular acupoint sticking can relieve the pain of patients after gynecological laparoscopic surgery,and the VAS score of patients decreased gradually within 48 hours after surgery.3.Auricular acupoints can reduce the incidence of' nausea and vomiting after gynecological laparoscopic surgery,promote the recovery of gastrointestinal function after surgery,and reduce the time of first exhaust and defecation after surgery.4.Auricular acupoint sticking can improve the satisfaction of patients after operation,which is worthy to get the clinical application.
