Objectives:To observe the effects of butylphthalide sequential therapy on neurological function and cognitive function in patients with acute cerebral infarction,meanwhile,TCD was used to explore the mechanism of butylphthalide sequential therapy on collateral circulation.Methods:Patients with acute cerebral infarction hospitalized in our department of neurology from November 2017 to November 2018 were randomly divided into observation group and control group.The control group received anti-platelet aggregation,statin lipid-lowering stable plaque and other conventional treatments after admission.Meanwhile,the treatment group was treated with butylphthalide sequential therapy on the basis of conventional treatment,including intravenous infusion of butylphthalide sodium chloride injection,twice a day,100 ml each time,for 14 days,then sequential taking butylphthalide soft capsules,0.2g/time,3 times/day,a total of 90 days.At admission(before treatment),use 7 days,14 days and 90 days,two groups of patients all evaluated NIHSS,MBI,MMSE and Mo CA scores.TCD was used to assess changes in collateral circulation counts,MCA Vm,and PI before and after treatment.Results:1.There was no significant difference in NIHSS and MBI scores between the two groups at the time of admission.The NIHSS scores on days 7,14,and 90 were lower than those on admission,and the MBI scores were higher than those on admission(P<0.05).The observation group was more significant(P<0.05).2.There were no significant differences in MMSE and Mo CA scores between the two groups at admission.Over time,the MMSE and Mo CA scores of the observation group before and after treatment were not significantly different from those of the control group(P>0.05).3.There was no statistical difference in the collateral circulation counts between the two groups before treatment(at admission).After treatment(on the 90 th day of treatment),the collateral circulation counts increased in both groups,but the difference between the two groups was not statistically significant(P>0.05).4.There was no significant difference in MCA Vm and PI between the two groups before treatment.The MCA Vm after treatment was higher than that before treatment,and the PI was lower than that before treatment(P<0.05).The MCA Vm and PI were significantly different from the control group(P<0.05).Conclusions: Butylphthalide sequential therapy may improve cerebral collateral circulation and increase cerebral blood flow in patients with acute cerebral infarction,which can improve the neurological function and improve the ability of daily living;Butylphthalide sequential therapy was not superior to the control group in improving cognitive function. |
