Objective: To explore the expression of CA?(carbonic anhydrase ?)in different types of renal cell carcinoma(RCC),analyzing the correlation between CA? and age,BMI,smoking and stage.To explore the clinical value of CA? as a specific tumor marker of renal cancer in diagnosis and prognosis.Methods: Collecting the postoperative pathological diagnosis for renal cancer radical renal clear cell carcinoma specimens of 60 cases,renal trauma,renal water line of unilateral nephrectomy post-operative pathological diagnosis for 30 cases of normal renal tissue samples another collection of 20 patients with relatively rare renal cell carcinoma specimens with renal papillary carcinoma,8 cases,7 cases of renal too color cell carcinoma,Bellini collecting duct carcinoma(3 cases),multilocular cystic renal cell carcinoma in 2 cases.Immuno histochemistry(SP)was used to detect the positive expression of CA? in pathological specimens of each group,and statistical methods were used to determine whether there were differences in expression among the three groups.In order to study the relationship between the positive expression of CA? and age,BMI,smoking and stage,60 cases of renal clear cell carcinoma were further divided into four subgroups according to the study factors.The expression of CA? in each subgroup was compared,and statistical analysis was conducted to draw a conclusion.Results: 1.The positive expression intensity of CA? in renal clear cell carcinoma,rare type renal carcinoma and normal renal tissue is shown in the following table.The positive expression rate is 85%(51/60),15%(3/20)and 0(0/30),respectively.The positive expression rate of renal clear cell carcinoma in group A was significantly higher than that of rare renal cell carcinoma in group B(85% > 15%),and the difference was statistically significant(P < 0.05).There was no positive expression of CA? in normal renal tissue of group C,which was significantly different from that of group A(P < 0.001).The positive expression of rare type of renal cell carcinoma ingroup B was not significantly different from that in group C(P=0.093).2.In the age group of 27~39 years old,8 out of 9 cases were positive,and the positive ratio was 88.9%.In the age group of 40~52 years,13 of the 16 cases were positive,and the positive ratio was 81.3%.Among the 21 patients aged 53-65 years,19 were positive,with a positive ratio of 90.5%.In the age group of 66-75 years,11 of the 14 cases were positive,with a positive ratio of 78.6%.CA ? expression was not significantly different in different age groups(P=0.573).Spearman rank correlation test was used to analyze the relationship between different age groups and CA ?positive expression intensity,and the test standard alpha =0.05 was used.The results showed that there was no significant correlation between CA? positive expression intensity and age(r=0.161,P=0.062).3.When BMI < 18.5,3 out of 6 patients were positive,with a positive ratio of 50%;When 18.5 BMI < 24,18 of the 23 patients were positive,and the positive ratio was78.3%.When 24 BMI < 27,20 of the 21 patients were positive,the positive ratio was95.2%.At BMI >,all the 10 patients were positively expressed,and the positive ratio was 100%.Spearman rank correlation test was used for analysis,and the results showed that the positive expression intensity of CA? was positively correlated with BMI(r=0.767,P < 0.05),indicating a good correlation.Further comparison of the four groups of data two USES 2 test and Fisher exact test can be concluded that: in the normal weight,overweight and obese patients CA? expression has no obvious difference(P > 0.05),but the weight of obese patients CA? patients with positive expression rate higher than that of lean body weight(100% > 50%),the difference statistically significant(P < 0.05).4.In this study,27 patients(45%)of the 60 patients with renal clear cell carcinoma smoked,including 24 males and 3 females.Among the 27 patients in the smoking group,23(85.2%)had positive CA? expression,while among the 33 patients in the non-smoking group,28(84.8%)had positive CA? expression.Further,2=1.92 and P > 0.05 can be calculated by four-grid table 2 test,indicating that smoking has no significant influence on CA? expression and the two are not correlated.5.?period of 18 patients with 17 express positive,positive rate is 94.4%;? period all express a positive result in 29 cases,positive rate of 100%;? period of 10 cases,4 cases only express positive,accounting for 40%;? period only 1 case in 3 cases with positive results,accounting for 33.3%.The late stage CA? positive expression rate is lower,and positive expression intensity during a ? periods and ? to + + +cases accounted for more than high,Spearman rank correlation test methods were analyzed,and the inspection standard of alpha = 0.05.Results: CA ? positive expression intensity was negatively correlated with tumor stage(r=-0.719,P < 0.05).Conclusion: CA? is highly expressed in clear cell renal cell carcinoma and there is no positive expressionn in normal renal tissue.In rare renal cell carcinoma,it's been proved as a very low level.CA? expression level is not significantly correlated with age and smoking,but positively correlated with BMI and negatively correlated with staging.CA? has potential value in the diagnosis,targeted therapy and prognosis evaluation of renal clear cell carcinoma.
