Objective: To investigate the effects of adenoidal hypertrophy with oral respiration on dental and maxillofacial development by cephalometric study of different degrees of adenoidal hypertrophy and different respiratory patterns.Methods: From February 2017 to April 2018,90 patients with different degrees of adenoidal hypertrophy who were seeking orthodontic treatment at the Second Clinical Medical College of Xinjiang Medical University were collected.Among them,60 cases had different degrees of adenoidal hypertrophy,30 cases had no breathing and adenoidal hypertrophy.According to the A/N ratio,they were divided into three groups,30 cases in each group.adenoid physiology hypertrophy group: 0.60<A/N?0.71;adenoid pathological hypertrophy group:A/N>0.71,and its pathological hypertrophy group can be divided into two groups:adenoid pathological hypertrophy with nasal breathing;adenoid pathological hypertrophy with oral breathing.Adenoid normal group: A/N<0.60.Adenoid size and dentofacial indicators were measured by Geometric Sketchpad 5.06 image processing software.The measured data were analyzed statistica lly.Results : Compared with the control group,the measurement items of ANS-Me/N-Me,?SNA,?SNB,NPg-SN,U1-NA,LI-NB,Y-axis and UI-SN were signifycantly different(P<0.05).The measurement items of GoGn-SN and NP-FH were significantly different between the normal group and the pathological hypertrophy group(P<0.05).Comparison of normal group with physiological hypertrophy group and patholog yical hypertrophy group,the MP-FH and LI-MP angles increased,and the pathological hypertrophy group had more significant difference.(P<0.05).The increase of LI-MP angle was more significant in pathological hypertrophy group(P<0.05).Comparing pathological hypertrophy with different respiratory modes,U1-L1,LI-MP and N Pg-FH measurement items had statistical differences(P<0.05),among which GoGn-S N increased,NPg-SN and Y-axis decreased,and the difference was more significant(P<0.05).Among the correlations between oral respiration and occlusal morphology,there were significant differences in U1-NA,LI-NB,GoGn-SN,FH-MP and LI-MP items(P<0.05),among which NPg-SN,U1-L1 and NPg-FH were negatively correlated(P<0.05).Conclusion: Adenoid hypertrophy and mouth breathing show mandibular retraction,lower central incisor lip inclination,and mouth breathing can lead toopening and closing.In clinical work,attention should be paid to the prevention and treatment of oral breathing,so as to achieve the stability of corrective effect.
