ObjectsDescribing the current situation of quality of life(QOL)of hypertension patients(HPs)under different socio-economic factors based on the national multicenter data.Comparing the gap between simple mean value and factor plus weight of QOL.Analyzing the factors that affect the QOL of HPs in each dimension.Comparing the similarities and differences of factors affecting the QOL of HPs in urban and rural areas. To provide evidence-based basis for improving the community standardized management of HPs and improving the health status and QOL of HPs,MethodsBased on literature research and related statistical methods, propensity score matching, multiple indicators multiple causes structure equation modelling(MIMIC SEM)and World Health Organization Quality of Life-BREF(WHOQOL-BREF)were used to describe the QOL of HPs under different socio-economic factors.28226 HPs came from 38 sub central communities,14 provinces, who were all in accordance with data integrity and the diagnostic criteria of hypertension.Propensity score matching(1:1)was made to urban and rural HPs;Analyzing the related factors affecting the QOL of different dimensions of HPs based on MIMIC SEM.ResultsThis study included in 28226 samples,10900 HPs and 17326 normal people, the average prevalence rate of hypertension was 38.6%.For the whole sample of HPs,ethnic factors were the most important factors affecting the QOL in physiological, psychological and environmental dimensions, and the corresponding influencing coefficients were 0.20,0.18 and 0.19.Education is the most important factor affecting the QOL in social relationship dimension. The influence eoefficient is 0.14;The influence direction of drinking on the QOL of HPs was different, the influence eoefficient of physiological dimension was-0.16,and the influence coefficient of psychological and social relationship dimension was 0.15 and 0.10,respectively.After propensity score matching(1:1),there were 9566 samples(4783:4783)meeting the requirement. For the sample HPs in cities, hypertension treatment is the most important factor affecting the physiological dimension, the influence coefficient is 0.26,and the nationality is the most important factor affecting the QOL in the psychological and environmental dimensions. The influence coefficients was 0.27.Whether there was hospitalization within one year was the most important factor affecting the dimension of social relations, and the influence coefficient was 0.23.For the sample HPs in rural areas, education is the most important factor affecting the quality of life in physiological, psychological and environmental dimensions, and the influencing coefficients are 0.28,0.25 and 0.20respectively.Marital status is the most important factor affecting the QOL in the dimension of social relations. The influence coefficient is-0,19.In physiological dimension, the influence coefficient of per capita GDP on urban HPs was 0.125 larger than that of rural HPs,and the influence coefficient of education status on rural HPs was 0.11 higher than that of urban HPs.In the psychological dimension, the influence coefficient of education on HPs in rural areas is 0.12higher than that of urban HPs.the influence coefficient of marital status on HPs in rural areas is-0.062 larger than that of HPs in urban areas. In the dimension of social relations, the influence coefficient of per capita GDP on urban HPs was 0.156 larger than that of rural HPs,and the influence coefficient of marital status on rural HPs was larger than that of urban HPs.In environmental dimension, the influence coefficient of education on HPs in rural areas is 0.08 larger than that in urban.ConclusionThere is a certain gap between the simple mean value and the weighted value of factors of QOL in HPs,among which the difference in the dimension of social relations is the largest. For the whole sample of HPs,ethnic factors are the most important factors affecting physiological, psychological and environmental dimensions, and education is the most important factor affecting the dimension of social relations.Drinking had a negative effect on the QOL in the physiological dimension of HPs,but had a positive effect on the QOL in the dimension of psychological and social relationship. In addition, sex, age, income status, hypertension grade, exercise are all important factors affecting the QOL of HPs.There were differences in the most important influencing factors and influencing coefficients of QOL in each dimension of HPs between urban and rural areas.Education is the most important factor affecting the physiological, psychological and environmental dimensions QOL in rural areas HPs.Hypertension treatment is the most important factor affecting the QOL in the physiological dimension of HPs in sample cities, and ethnic difference is the most important factor affecting the QOL in psychological and environmental dimensions of HPs in sample cities.Whether there is hospitalization within one year is the most important factor affecting the QOL in social relationship dimendion of HPs in the sample city.In addition, the type of medical insurance is an important factor affecting the QOL of HPs in rural areas.
