Purpose:To study the optimization effect of shear wave elastography quantitative parameters on BI-RADS category of breast lesions.Methods:Before biopsy or surgery,76 patients(85 breast lesions)underwent BI-RADS categorization and shear wave elastography test,in which we obtained several shear wave elastography quantitative parameters(AEmax,AlEmax,A2Emax and A3Emax).Combined with the pathology results,the differences of quantitative parameters in benign and malignant breast lesions were compared.The ROC curve was constructed with AEmax,AlEmax,A2Emax and A3Emax as test variables and pathological results as gold standard,geting the cut-off value of the parameter of the best diagnostic performance in order to optimize the BI-RADS categorizationas.The diagnostic efficiency,sensitivity,specificity,positive predictive value,negative predictive value and diagnostic accuracy before and after BI-RADS classification optimization were compared respectively.Results:The values of AEmax,Shell lEmax,Shell2Emax and Shell3Emax were significantly greater than those in the benign group(P<0.05),with the maximum AUC value of She113Emax(0.854)and the diagnostic threshold of 101.25 kPa.The optimized BI-RADS categorization efficiency and sensitivity were significantly higher than conventional ultrasonic BI-RADS categorization method(P<0.05).There were no statistically significant differences regarding the specificity,positive predictive value,negative predictive value and accuracy of BI-RADS categorization before and after optimization(P all>0.05).Conclusion:The shear wave elastography quantitative parameter She113Emax couldsiginificantly improve the ultrasonic BI-RADS categorization.
