Objective:The purpose of our study was to evaluate the value of Shear wave elastography technology in differentiating benign from malignant lesions of the breast.Methods:This study include 87 patients and eventually get the pathologic results of 121 lesions as the research object,including 2 men(one lesion either)and 85 women(119lesions).Routine uitrasound examination observe lesions of the shape,orientation,edge,inner echoes,calcification,rear echo characteristics and blood flow signal of two-dimensional,and carries on the classification of BI-RADS,then to shear wave elasticity examination,measuring the mass of the internal and surrounding tissue elastic modulus parameters,include SD,Emax,Emean,Emin,Eratio and record the color overlay features,the area under the ROC curve obtained by calculation of elastic modulus parameters optimal diagnostic critical value.Conclusion:(1)89 breast benign lesions and 32 malignant lesions,conventional ultrasound in shape,location,margin,border,internal echo,calcification,peripheral hyperechoic halo,rear echo,mass internal and peripheral blood flow signal differences are statistically significant.(2)Under the SWE,the tissue surrounding and inside the breast malignant tumor of the modulus of elasticity parameters of SD,Emean,Emax,Eratio significantly greater than the benign tumors(P < 0.001).Emax in the internal and surrounding tissue elastic modulusparameters is the best area under the ROC curve,respectively 0.941,0.911,its diagnosis threshold respectively take 78.7 Kpa and 84.2 Kpa,diagnosis of best performance.(3)Under the SWE,the size of malignant breast lesions are greater than the size under the conventional ultrasound,and closer to the size of histology compared with conventional ultrasonic.The best diagnostic point color elastic pattern classification for mode 2,3.(4)The conventional ultrasonic BI-RADS classification combined with surrounding tissue elastic modulus parameter Emax and SWE colour overlay features have the best diagnosis performance,the sensitivity was 97.8%,88.7%,the diagnostic performance compared to the BI-RADS classification increases,and have the higher specificity.Conclusion:SWE elastic modulus parameters and colour overlay features classification for evaluation of benign and malignant breast lesions added new diagnostic parameters,and the conventional ultrasonic BI-RADS classification combined with surrounding tissue elastic modulus parameter Emax and SWE colour overlay features have the best diagnosis performance,provides a new diagnostic basis.
