Objective:1.The objective of this' study was to assess the accuracy of 3D computer-aided surgery simulation by using Dolphin Imaging 3D software for soft and hard tissue prediction of orthognathic surgery in the mandible.2.The study was performed to analyze different points between predicted 3D image and real 3D image after orthognathic surgery in the mandible.These results will guide the clinician's surgical operation,postoperative facial prognosis,and improve the effectiveness of communication with patients.Material and methods:Eleven patients with skeletal malocclusion treated by combined surgical and orthodontic at the Affiliated Stomatological Hospital of Zhejiang University School of Medicine from 2016 to 2018 were selected as subjects.The surgical sites of subjects were limited to the mandible.Cone beam computed tomography(CBCT),3D photographs and oral scan data were taken before surgery average 6 months after surgery.8 skeletal Landmarks and 8 soft tissue landmarks were selected:B?Pog?MfR?MfL?GoR?GoL?CoR?CoL;Sn?UL?Li?Pog'?AcR?AcL?ChR?ChL.The distances from each marker to three planes were measured in the rectangular coordinate system,the results paired t-test for dependent sample and correlation analysis.The surface to surface model applied by Geomagic Studio 12 software makes use of an automated comparison of the virtual plan and the actual outcome.Results:1.The prediction accuracy is satisfactory in mandible after orthognathic Surgery using Dolphin imaging 3D software.Among the 8 landmarks,24 measure distances in simulated and postoperative real data only GoR,CoR,CoL showed differences,P<0.05.There were no significant changes in the remaining points(P>0.05).2.The predictability of soft tissue is accurate using Dolphin imaging 3D software.Among the 8 selected markers and 24 measure distances,the difference between surgical simulation and postoperative real data in Pog' and AcR markers was statistically significant(P<0.05).There were no significant differences in the remaining landmarks(P>0.05).3.There was no difference in the accuracy of hard tissue prediction and soft tissue by using Dolphin imaging software.Conclusion:The 3D computer-aided surgery simulation based on Dolphin imaging software has high accuracy for soft and hard tissue simulation of orthognathic surgery which limited in the mandible.This software can obtain accurate and predictable surgical results and has directed the significance of clinical treatment and diagnosis.However,the deviation of bilateral condyles could not fulfill.This software has important clinical significance for the diagnosis of dentomaxillofacial deformities,the establishment of orthodontic target position,surgical operation,postoperative facial pattern prediction and doctor-patient communication.
