Objective To study the correlation between the characteristic parameters of photoplethysmography(PPG)and severity of coronary artery lesions and the predictive value of the characteristic parameters of PPG for coronary heart disease.Methods Three hundred and thirty-eight patients who underwent coronary angiography(CAG)or coronary computed tomography angiography(CCTA)in the Chinese PLA General Hospital from August 2018 to November 2018 were divided into the coronary heart disease group(226 cases)and the non-coronary heart disease group(112 cases)according to their CAG or CCTA.The coronary heart disease group was further divided into the single-vessel lesion group(55 cases),the double-vessel lesion group(74 cases)and the multi-vessel lesion group(97 cases).The general clinical data of the patients were collected.The PPG detection was performed to extract characteristic parameters stiffness index(SI),reflection index(RI),crest time(CT),normalized crest time(NCT)and crest time ratio(CTR),K,K1,K2 and K1/K2.The differences of SI,RI,CT,NCT,CTR,K,K1,K2 and K1/K2 between CHD group and non-CHD group were compared,and the correlation between PPG characteristic parameters and the severity of coronary artery lesions was analyzed.Results There were significant differences in gender,age,history of hypertension,history of diabetes,smoking history,HDL-C and creatinine between the patients of CHD group and non-CHD group(P<0.05).There was no significant difference in BMI,history of stroke,history of hyperlipidemia history of drinking,TC,TG,LDL-C,LVEF and LVEDD between the patients of CHD group and non-CHD group(P>0.05).The characteristic parameter SI of PPG in CHD group was significantly higher than that in non-CHD group(6.7478±1.0331m/s,6.3876±1.1224m/s,respectively,P=0.004).There was no significant difference in other PPG parameters between the two groups(P>0.05).Multivariate Logistic regression analysis of CHD showed that male,age,diabetes history,smoking history and SI were risk factors for CHD,and HDL-C was a protective factor for CHD.The area under the ROC curve predicted by the Logistic regression equation model was 0.836.There was a statistically significant difference in the characteristic parameter SI of PPG among the three subgroups in the CHD group(P=0.024).There was a statistically significant difference in SI between the single-vessel lesion group and the multi-vessel lesion group(P<0.05),but no statistically significant difference was found between the single-vessel lesion group and the double-vessel lesion group,or between the double-vessel lesion group and the multi-vessel lesion group(P>0.05).There was no significant difference in other PPG characteristic parameters among the three groups(P>0.05).SI was positively correlated with the severity of coronary artery lesions(r=0.162,P=0.015),and other PPG characteristic parameters were not significantly correlated with the severity of coronary artery lesions.Conclusion The characteristic parameter SI of PPG is an independent risk factor of CHD.It has a certain correlation with the severity of coronary artery lesions,and certain value in the evaluation of the severity of coronary artery lesions.The multivariate Logistic regression equation model including gender,age,diabetes history,smoking history,SI and HDL-C has certain value for the prediction of CHD.
