Preliminary Application Of Endoscopic Assisted Technique In The Resection Of Benign And Malignant Breast Tumors

Objective: The benign and malignant breast tumors were resected by endoscopic assisted surgery,and the clinical effect of endoscopic assisted suigical resection of breast tumor was preliminarily evaluated,and the safty and feasibility of endoscopic assisted breast surgery were discussed.Methods:1.From September 2016 to January 2019,11 patients(all of whom performed the surgery according to the patients' wishes)who underwent endoscopic-assisted breast surgery in the Department of Breast and Thyroid,the First Affiliated Hospital of the University of South China,were selected as study subjects,there were 8 cases of fibroadenomas,including 1 case of fibroadenoma with ductal epithelial dysplasia and 1 case of fibroadenoma complicated with adenopathy,1 case of double breast augmentation emulsion-induced pseudotumor.Twenty patients with benign tumors who underwent open surgery at the same time were selected as the control group to observe the clinical effects of endoscopy-assisted surgery for benign breast tumors.2.Recalling 3 patients who underwent endoscopic-assisited of breast malignant breast-conserving surgery for breast cancer in the Department of Breast and Thyroid of the First Affiliated Hospital of the University of South China,were selected as research subjects,from September 2016 to January 2019.The surgical methods were all patients' wishes.Three of the cases were found to be invasive ductal carcinoma.The surgical methods and postoperative conditions described in detail.Complications were observed and wound healing was evaluated 2 weeks after the surgery.Six months after the operation,questionnaire survey was used to evaluate the subjective satisfaction of patients,and Zejing method was used to evaluate the objective satisfaction of patients.Patients were followed up for tumor recurrence and metastasis.Results:1.In the resection of benign breast tumors,compared with the open surgery group,the endoscopic surgery group was slightly longer in the operation time,the difference was statistically significant(P<0.05),but the intraoperative blood loss,postoperative pain degree,and hospitalization days were significantly less than the open group,and the postoperative length of scar was significantly shorter,all the difference was statistically significant(P<0.05).During the postoperation follow-up,all patients in the endoscopic surgery group had excellent breast aesthetic effect,which was significantly higher than that of the open group(95%),and the satisfaction of patients in the endoscopic surgery group(90.91%)was also significantly higher than that in the open group(65%),the difference was statistically significant(P<0.05).There was no statistically significant difference between the two groups in age,BMI,hospitalization costs,distance from the areola,mass diameter and mass location(P>0.05).No postoperative complications were observed in the two groups,and the wounds healed in grade A.2.Malignant breast tumor: One patient presented breast skin ecchymosis after surgery,followed by no necrosis of the skin,wound healing,no sensory disturbance.The remaining 2 patients had a grade A wound healing,no skin necrosis,sensory disturbance.Six months after surgery,3 patients all felt that the location of scar on the breast surface was smaller and hidden than that of traditional open surgery,with beautiful appearance of breast and no obvious change in size.The patients were evaluated by Zejing method: excellent: 2 cases;Good: 1 case;General: 0 cases;poor: 0 cases.No tumor recurrence or metastasis was found in the patients who underwent regular reexamination every 3 months.Conclusion:Endoscopic assisted breast surgery has ceitain cosmetic advantages in benign tumors and is a promising technique.However,it still needs to be improved in terms of operation time and economic indicators,and its application in breast malignant tumors to be further explored.
