Objective:To investigate the correlation among blood pressure variability(BPV),pulsatility index(PI)and cognitive impairment in patients with cerebrovascular disease(CSVD),and to provide theoretical basis for finding a method to prevent the occurrence and development of cerebrovascular disease.Methods:According to the inclusion and exclusion criteria,60 patients with cognitive impairment of cerebrovascular disease diagnosed in our hospital from August 2016 to January 2019 who met the requirements were collected as the cognitive impairment group,and 43 patients with cognitive impairment of cerebrovascular disease whose age and gender matched the case group were selected as the control group.All patients completed Biochemical tests including blood lipids,blood glucose and so on,head MRI,24-hour ambulatory blood pressure monitoring(24hABPM),MOCA and transcranial doppler(TCD).Compared basic data,PI and blood pressure standard(SD)of this two groups.The relationship between PI and BPV was analyzed by partial correlation;The relationship between PI,BPV and the score of MOCA was analyzed by Pearson correlation,and influence factors of affecting the occurrence of cognitive impairment by Logistic regression analysis,and P<0.05 was considered statistically significant.Results:(1)There was no statistical difference between the cognitive impairment group and the control group in age,gender,History of coronar y heart disease,smoking history,drinking history,triglyceride,low density lip oprotein cholesterol,total cholesterol,uric acid and homocysteine(P?0.05);T he number of years of education in cognitive normal group were higher than that in the cognitive impairment groupand the difference was statist ically significant(P<0.05).(2)PI,daytime systolic blood pressure Standard deviation(dSBPSD),daytime diastolic blood pressure standard deviation(dDBPSD),nighttime diastolic blood pressure standard deviation(nSBPSD),24 hour diastolic blood pressure standard deviation(nDBPSD),24 hour d iastolic blood pressure standard deviation(24hDBPSD)in cognitive impa irment group were higher than that in the normal control group(P<0.05)and the differences were statistically significant.(3)The score of MOCA was negatively correlated with 24 hSBPSD,24hDBPSDdSBPSD dDBPSD,n SBPSD,nDBPSD,and PI in patients with CSVD(r,respectively,-0.369,-0.403,-0.364,-0.381,-0.262,-0.243 and-0.526,P<0.05).(4)using partial correlati on analyzing the correlation of PI and BPV,the results show that there was no correlation between the PI level and the BPV indicators(P>0.05).(5)Using cognitive impairment as the dependent variable,and PI,24 hSBPSD,24hDBPSD,dSBPSD,dDBPSD,nSBPSD,nDBPSD as the independent variables,the result shows that pland nSBPSD are the independent influ ence factors of cognitive dysfunction(P<0.05).Conclusion:Blood pressure variability and PI may be associated with cognitive impairment in patients with cerebrovascular disease.PI and nSBPSD may be risk factors for cognitive impairment in cerebrovascular disease.
