Correlation Among The TRACK Score,Tital Breathing Pulmonary Function And Exhaled Nitric Oxide In Asthmatic Children Under 5 Years Old

Objective: Tidal breathing pulmonary function and Fractional exhaled nitric oxide are widely used as objective indicators in the asthma control level for children under 5 years old.The test for respiratory and asthma control in kids(TRACK)is the only quantitative evaluating tool of asthma control level by GINA guidelines for children with asthma under 5 years old.The correlation of the above indicators is less research in China.In the present study,it is focused on the changes of the test for respiratory and asthma control in kids.Tidal breathing pulmonary function and Fractional exhaled nitric oxide(FeNO)in acute stage and remission stage of asthmatic children.And investigate their correlation to verify their clinical value in asthma.Methods:(1)A total of 63 children with asthma under 5 years old who were admitted to the pediatric department of the Second Hospital of South China University from December 2017 to December 2018 were enrolled.36 males and 27 females,aged 2.3~4.8 years old;At the same time,the children in the physical examination were used as the control group,14 males and 13 females,aged 2.5~4.8 years old;(2)All children completed to the lung function test,determination of Fractional exhaled nitric oxide(FeNO)and TRACK test.(3)The difference between the lung function index and level of FeNO in children with asthma and control group were analyzed.(4)The difference between the TRACK scores in asthmatic children with acute stage and remission stage was analyzed.(5)The correlation between TPTEF/TE,VPEF/VE and level of FeNO in the asthma group was analyzed by Pearson correlation analysis.(6)The correlation between TPTEF/TE,VPEF/VE and TRACK score in the asthma group was analyzed by Pearson correlation analysis.(7)The correlation between FeNO level and TRACK score in the asthma group was analyzed by Pearson correlation analysis.Results:(1)The level of FeNO in the acute stage,remission stage and control group were(27.76±10.86)ppb,(23.86±8.07)ppb,(10.37±5.35)ppb,respectively.The difference was statistically significant(P<0.001);(2)The TPTEF/TE in the acute stage,remission stage,and control groups of the asthma group were(19.66±4.86)%,(31.81±7.78)%,(37.26±4.94)%,respectively,and the difference was statistically significant(P<0.001);(3)The VPEF/VE in the acute stage,remission stage,and control groups of the asthma group were(19.89±4.86)%,(32.48±7.86)%,(38.18±5.81)%,respectively,the difference was statistically significant(P<0.001);(4)The TRACK scores in the acute stage and remission stage were(44.29±11.06)and(71.19±13.43),respectively,and the difference was statistically significant(P<0.001).(5)The Pearson correlation coefficient of TPTEF/TE,VPEF/VE and FeNO levels in the acute stage of asthmatic group was r=0.034,r=0.020(P>0.05),respectively,and there was not a correlation between TPTEF/TE,VPEF/VE and FeNO.(6)The Pearson correlation coefficients of TPTEF/TE,VPEF/VE and FeNO levels in the remission stage of asthmatic group were r=-0.010,r=-0.050(P>0.05),respectively,and there was not a correlation between TPTEF/TE,VPEF/VE and FeNO.(7)The Pearson correlation coefficients of TPTEF/TE,VPEF/VE and TRACK scores in the acute of asthmatic group were r=0.788,r=0.792(P<0.05),respectively,with significant positive correlation.(8)The Pearson correlation coefficients of TPTEF/TE,VPEF/VE and TRACK scores in the asthma group were r=0.947,r=0.964(P<0.05),respectively,and all had significant positive correlation.(9)The correlation coefficients between TRACK score and FeNO level in acute and remission stage of asthmatic children were r=-0.042,r=-0.098(P>0.05),respectively,and there was not a correlation between TRACK score and FeNO.Conclusions: 1.TRACK score was positively correlated with tidal pulmonary function(TPTEF/TE,VPEF/VE),but not with FeNO.TRACK can reflect the condition of children under 5 years old with asthma,which is reliable and simple.2.The pulmonary function index(TPTEF/TE,VPEF/VE)can reflect airway obstruction in children under 5 years old with asthma.3.FeNO could directly reflect airway inflammatory degree.
