Clinical Analysis Of 45 Cases Of Plastic Bronchitis Treated By Bronchoalveolar Lavage

Objective To explore the clinical value of bronchoalveolar lavage in the treatment of plastic bronchitis in children.Methods Retrospective analysis of the cases of children diagnosed with PB from January,2012 to March,2018 in Chenzhou Hospital of South China University,45 cases of plastic bronchitis are perfect bronchoalveolar lavage treatment and alveolar lavage 7 kinds of respiratory viral antigen detection,alveolar lavage mycoplasma pneumoniae nucleic acid detection,lavage pathogenic bacteria cultivation.The critical score,arterial oxygen partial pressure,arterial oxygen saturation and lung imaging data of children before and after bronchoalveolar lavage were compared.The etiological characteristics of bronchoalveolar lavage fluid were analyzed.Results1.The critical score,arterial oxygen partial pressure?PaO₂?,arterial oxygen saturation?SaO₂?and lung imaging improvement were compared before and after 1 time of bronchoalveolar lavage.The critical score,oxygen partial pressure,arterial oxygen saturation and lung imaging were all improved,and the difference was statistically significant?P<0.05?.2.After multiple bronchoalveolar lavage,children's critical score,arterial oxygen partial pressure?PaO₂?,arterial oxygen saturation?SaO₂?and pulmonary imaging improvement were compared with those before alveolar lavage,and the results were significantly improved,with statistically significant differences?P<0.05?.3.The critical score,arterial oxygen partial pressure?PaO₂?,arterial oxygen saturation?SaO₂?,and pulmonary imaging improvement rate of the children after multiple bronchoalveolar lavage were significantly improved compared with the control group after 1 lavage,and the difference was statistically significant?P<0.05?.4.45 cases with plastic bronchitis were analyzed.The pathogeny examination of alveolar lavage fluid showed that 14 cases of MP could develop in four seasons.There were 5 cases of influenza a virus?IFA?,5cases of influenza b virus?IFB?and 5 cases of adenovirus?AVD?.Among them,there were 1 case of AVD complicated with respiratory syncytial virus infection and 4 cases of bacteria?1 case of IFA,1 case of IFB combined with pneumococcal infection?,and the remaining 14 cases were found to have no clear etiology,and occurred throughout the year.Conclusion1.The bronchoalveolar lavage?BAL?is a direct,safe and effective technique for the diagnosis and treatment of plastic bronchitis,which can significantly improve critical score of children and improve ventilation and pulmonary imaging in children as soon as possible.2.The detection of bronchoalveolar lavage pathogen is significant for the diagnosis and treatment of plastic bronchitis.
