The Clinical Study Of On The Efficacy Of The Applicat Ion Of Jiao-qi Powder On The Hidden Blood Loss Following Femoral Neck Fractures Treated By Hemiarthroplasty

Objective:The aim of the study is to examine the efficacy of the application of Jiao-qi powder on the hidden blood loss(HBL)following femoral neck fractures treated by hemiarthroplasty.Patients and Methods:A total of 78 patients with femoral neck fractures were allocated randomly from January 2018 to January 2019 in the First Affiliated Hospital of Guangzhou University of Chinese Medicine.Strictly abide by the exclusion criteria and principles of randomize control,these patients were divided into two groups,the treatment group 40 cases(Group A)and the blank control group 38 cases(Group B).Both groups were treated with hemiarthroplasty,and all patients should be comprehensively examined properatively as well as accepted the same routine peri-operative protocol.After admission,group A started to take oral Jiao-qi powder(Gelatin 6g,Panax notoginseng 6g,bid)since the next day,lasting until the discharging day.None oral Chinese medicine applied in group B.All indicators,with basic diseases,received symptomatic treatments.After the operation,the patients were checked regularly and relevant observation indexes were recorded.The total blood loss and hidden blood loss were calculated according to the calculation formula.Statistical analysis was conducted according to the results and conclusions were drawn.Results:In terms of general data such as age,sex,height,BMI,operation time,fractures classification,pre-operative blood analysis and pre-operative cogulation index and so on,there were no statistical difference between Group A and Group B.Total blood loss(TBL)of both groups(Group A 646.76±123.34mL vs Group B 699.26±124.97mL,P=0.066>0.05),and hidden blood loss(HBL)(Group A 456.04±128.12mL vs Group B 493.52±138.81mL,P=0.219>0.05),it shows that Group A was lower than Group B on both parameters on average,but there was no statistical difference.Comparison of perioperative hemoglobin(HGB)changes between the two groups:preoperative HGB(group A 121.53±15.00g/l.vs119.82±15.58 g/L,P=0.623>0.05),the first day after surgery(P0D1)(group A 104.60±13.31g/L vs B group 98.92±12.73 g/L,P=0.058>0.05),the third day after surgery(POD3)(group A 102.78±14.78 g/LvsB group 97.05±10.60 g/L,P=0.049<0.05),the seventh day after surgery(P0D7)(group A 108.15±13.54 g/LvsB group 103.11±5.53 g/L,P=0.034<0.05).The postoperative HGB mean level of group A at each time point was higher than that of group B,and the difference in HGB on the first day after surgery were not statistically significant(P>0.05).The difference in HGB on the third day and the seventh day after surgery was statistically significant(P<0.05).Statistical analysis of transfusion rate or postoperative complications in the two groups showed no statistical significance(P>0.05).Conclusion:The application of,Jiao-qi powder may lead to smaller post-operative decrease in the level of hemoglobin(HGB)in a certain extent,and hemoglobin decreased to the lowest level on the third day after surgery and seems to recover on the seventh day after surgery.It can' t effectively reduce the perioperative hidden blood loss and total blood loss,but it shows that the treatment group has a less amount of hidden blood loss and total blood loss in the mean level of both parameters.Maybe Jiao-qi powder could be an auxil iary treatment measures of hidden blood loss for femoral neck fractures paintients with hemiarthroplasty and further researches are needed.
