ObjectiveSepsis is a critical disease threatening human life and health.Despite the progress of clinical research on sepsis,the clinical morbidity,mortality rate and medical expense of sepsis are still high.TCM holds that deficiency of healthy-qi is the internal factor of the occurrence and development of sepsis.Acute deficiency as the key pathogenesis of severe sepsis runs through the whole process of the occurrence and development of sepsis,and Fu-Zheng-Gu-Ben method should be ta ken as the core of the treatment of sepsis.This study explored the effect of Fu-Zheng-Gu-Ben method on clinical efficacy of sepsis patients assisted by routine western medicine treatment,in order to provide more ideas and references for clinical treatment of sepsis with Fu-Zheng-Gu-Ben method.MethodsA prospective study was conducted to include sepsis patients who were diagnosed in the Ersha Branch Emergency Department and Fangcun Branch ICU of the Guangdong Provincial Hospital of Traditional Chinese Medicine from June 2017 to June 2018 in accordance with the diagnostic criteria of Sepsis 3.0 and hospitalized.All patients were randomly divided into the conventional western medicine treatment group and the experimental group according to the ratio of 2:1.Among the experimental group,according to TCM syndrome differentiation,if the patients are Yang violent syndrome,their treatment plan is to add Shenfu injection to the routine Western medicine treatment.If the patients are Yin exhaustion syndrome,their treatment plan is to add Shengmai injection to the routine Western medicine treatment.In this way,the experimental group was divided into the Shenfu group and the Shengmai group.The three groups of treatments were all 7 days.Three groups of SOFA scores,APACHE? scores,D-dimer,platelet count(PLT),white blood cell count(WBC),procalcitonin(PCT),C-reactive protein(CRP)and TCM syndrome changes(consciousness,skin temperature,skin color,tongue,pulse,etc.)were collected before and after treatment.And follow-up records of 28-day mortality rate and days of hospitalization.Statistical analysis was performed on the differences between the indicators before and after treatment and between groups using SPSS 17.0 software.The test level a was 0.05,andP<0.05 indicated that the difference was statistically significant.Results1.A total of 73 patients with sepsis who met the criteria were included in the study,including 24 in the Shenfu group,25 in the Shengmai group,and 24 in the conventional group.There were no significant differences in age,gender,primary disease distribution,heart rate,blood pressure,respiratory rate,peripheral blood glucose,and peripheral oxygen saturation between the three groups(P>0.05).The baseline characteristics of the three groups were homogeneous and comparable.2.Compared with before treatment,SOFA score,APACHE ? score,D-dimer,WBC,CRP and PCT decreased significantly on the 3rd and 7th day after treatment in Shenfu group?Shengmai group and conventional treatment group,PLT increased significantly.The differences between the groups were statistically significant at different time points before and after treatment(P<0.05).3.Compared with the conventional group,the D-dimer and WBC values of the Shenfu group and the Shengmai group were lower than those of the conventional group on the third day after treatment(P<0.05);On the 7th day after treatment,the SOFA score,APACHE ? score,D-dimer,WBC and PCT values of Shenfu group and Shengmai group were lower than the conventional group(P<0.05);However,there was no significant difference in SOFA score,APACHE? score,D-dimer,PLT,WBC,PCT and CRP at each time point between the Shenfu group and the Shengmai group(P<0.05).4.Although the days of hospitalization in the Shenfu group and the Shengmai group were significantly lower than that in the conventional group(<0.05),there was no significant difference in the 28-day mortality rate and the Chinese medicine treatment effect between the three groups(P>0.05).ConclusionOn the basis of conventional treatment of Western medicine,Shenfu injection or Shengmai injection can improve the blood coagulation function of patients with sepsis,effectively regulate the inflammatory response of the body during sepsis.Finally,the patient' s SOFA score and APACHE ? score were reduced.And the two drugs shortened the number of hospital stays.However,this study did not find that the Fu-Zheng-Gu-Ben method has an impact on the prognosis of patients.The clinical indications and application timing of the two drugs need further study and discussion.
