ObjectiveTo observe the clinical effect of Xueshuantong injection of traditional Chinese medicine on the prevention of lower extremity deep venous thrombosis in icu patients.MethodsA prospective randomized controlled clinical.trial was conducted from January 2018 to December 2019,80 patients with ICU patients who were newly treat ed in the intensive care unit of Zhongshan traditional Chinese Medicine Hospital affiliated to Guangzhou University of traditional Chinese Medicine were randomly divided into experimental group(n=40)and control group(n= 40).Both groups were treated with primary dissease treatment and the same basic preventive measures.The control group was subcut aneously injected with low molecular weight heparin sodium injection and the experimental group was given intravenous drip of Xueshuantong for 5 days.The results of plasma D-dimer,hemorheology and color Doppler ultrasonography of deep vein vessels in both lower extremities were observed and compared before treatment and on the 3rd and 5th day after treatment.at the same time,adverse reactions or events were recorded.The data were statistically analyzed to objectively evaluate the efficacy and safety of Xueshuantong for injection.Results1.A total of 72 ICU patients were enrolled in and completed the clinical trial,including 36 patients in the experimental group and 36 patients in the control group.there was no significant difference between the two groups in age,sex,primary disease,APACHE Ⅱ score and other general data(P>0.05),which indicated comparability between the two groups.2.Before treatment,there were no significant differences in D-dimer,whole blood viscosity(high and low shear),plasma viscosi ty,hematocri t,erythrocyte aggregation index and erythrocyte deformability index between the two groups(P>0.05).After treatment,In both groups,D-dimer,whole blood viscosity(high and low shear),plasma viscosity,hematocrit,erythrocyte aggregation index decreased(P<0.01),and erythrocyte deformability index increased(P<0.01).D-dimer,whole blood viscosity(high and low shear),plasma viscosity,hematocrit and erythrocyte aggregation index decreased with the passage of time.The difference was statistically significant(P<0.01),and the erythrocyte deformability index increased with the passage of time(P<0.01).3.The erythrocyte deformability index of the experimental group was not significantly higher than that of the control group on the 3rd day after treatment(P>0.05),but higher than that of the control group on the 5th day after treatment(P<0.01).The level of D-dimer in the control group was lower than that in the experimental group on the 3rd and 5th day after treatment(P<0.01).The whole blood viscosity(high and low shear),plasma viscosity,hematocrit and erythrocyte aggregation index in the experimental group were not significantly different from those in the control group on the 3rd and 5th day after treatment(P>0.05).4.Before treatment,no thrombosis was found in both groups by color Doppler ultrasonography of the deep veins of the lower extremities,two groups of patients can be considered to be comparable in terms of LDVT.On the 5th day after treatment,the results of color Doppler ultrasound showed that the incidence of LDVT in the treatment group was 8.57%,which was similar to that in the control group(5.41%)(P>0.05).ConclusionTraditional Chinese medicine of Xueshuantong injection can improve blood hypercoagulability,reduce blood viscosity,improve microcirculation,has a good preventive effect on LDVT in patients with ICU patients,and less adverse reactions,which is worthy of clinical recommendation.
