Objective: To observe the effects of transcatheter left gastric artery embolization on body weight,leptin,ghrelin protein expression and related biochemical indexes in obese beagle dogs with type 2 diabetes mellitus(T2DM).To determine the effect of left gastric artery embolism on endocrine factors in Beagle dog model.Methods: Based on the principle of randomized controlled design,this study took Beagle dog as the research object.1)establishment of type 2 diabetes model: 9 healthy adult male beagle dogs were randomly divided into two groups: the treatment group and the control group.the treatment group was fed with a special high-fat and high-sugar diet for 4 weeks,intraperitoneal injection of 25 mg/kg sodium citrate solution of streptozotocin(STZ),followed by continuous feeding with high-calorie diet until 10 weeks.the control group was fed with normal food.after the model of type 2 diabetes model was successfully established,the experimental group was divided into embolic group and non-embolic group by a completely random method.Three Beagle dogs were needed for each subgroup: Group A: type 2 diabetes embolism group,transcatheter left gastric artery polyvinyl alcohol drug particle(PVA)embolism group;Group B: Type 2 diabetes without embolism group was perfused with 12 ml of normal saline through catheter left gastric artery.Group C: The control group was perfused with 12 ml of normal saline through the catheter left gastric artery.Ten weeks after the operation,the animals were killed,the serum was extracted and the expression levels of leptin,hunger hormone and insulin were detected by Elisa.At the same time,the animal body weight,blood sugar and related biochemical indexes were measured weekly and analyzed statistically.2)Establishment of simple obesity model: 9 healthy adult male Beagle dogs were randomly divided into two groups: experimental group and control group.The experimental group was given special high-energy feed,while the control group was given ordinary conventional feed.After the obesity model was successfully established,the experimental group was divided into embolized group and non-embolized group based on whether or not embolism occurred.Three beagle dogs were needed for the experiment in each subgroup: group A: obese embolism group,polyvinyl alcohol particle(PVA)transcatheter gastric left artery embolism group;Group B: Obese group without embolism was perfused with 12 ml of normal saline through the catheter left gastric artery.Group C: the control group was perfused with 12 ml of normal saline through the catheter left gastric artery.Ten weeks after the operation,the experimental animals were killed,the gastric tissue proteins were extracted,and the expression levels of leptin and ghrelin were detected by western blot.At the same time,the body weight,diet and related biochemical indexes of the animals were measured every two weeks after the operation,and finally statistical analysis was carried out.Results: 1)Elisa showed that the expression of ghrelin in the obese and T2 DM embolized group(5.35±0.094)and the non-embolized group(11.24±0.16)was significantly lower than that in the non-embolized group(P<0.05).the expression of ghrelin in the embolized group and the normal control group(7.705 0.09)was significantly lower than that in the non-embolized group(P<0.05),and the concentration of ghrelin after operation was reduced.(2)the expression of leptin in the obese and T2 DM embolic group(7.43±0.11)was significantly higher than that in the non-embolic group(15.40± 0.07)(P<0.05).The leptin expression in the embolic group was significantly higher than that in the normal control group(9.38±0.11)(P<0.05).The expression level(17.72±0.1298)of serum insulin in the obese and T2 DM embolic group(19.55±0.2274)was significantly higher than that in the non-embolic group(P<0.05).the insulin concentration in the experimental group was significantly higher than that in the normal control group(20.89±0.1144)(P<0.05).The expression of insulin resistance in obese patients with T2 DM embolism group(12.57±0.25)was significantly higher than that in non-embolism group(7.260±0.13)(P<0.05).The expression of insulin resistance in embolism group was significantly higher than that in normal control group(5.015±0.17)(P<0.05),and insulin resistance was improved after operation.2)Western blot showed that the expression of ghrelin in simple obesity embolism group(0.7846±0.4184)was significantly different from that in non-embolism group(2.31±0.26)(P<0.05).The expression of ghrelin in treatment group was significantly different from that in normal control group(1.07±0.30)(P<0.05),and the expression of ghrelin was significantly decreased after operation.The expression of leptin protein in simple obesity embolism group(0.66±0.14)was significantly different from that in non-embolism group(2.15±0.24)(P<0.05),and was significantly different from that in normal control group(1.20± 0.07)(P<0.05).the expression level of leptin protein was significantly decreased after operation.Conclusion: Left gastric artery embolization can effectively inhibit the expression of leptin and ghrelin protein in tissues and cause weight loss.Transcatheter left gastric artery embolization can significantly reduce leptin and ghrelin concentrations in obesity,obesity with type 2 diabetes.Transcatheter left gastric artery embolization plays a positive role in reducing blood sugar,improving insulin resistance and increasing insulin level in obese beagle dogs with type 2 diabetes.
